United Way of Lancaster County has evolved from its roots as a fundraising organization to a critical community convener that connects nonprofits, businesses, community leaders, public officials, and community residents to expand opportunities for people to succeed.

Lancaster County’s BOLD Example

The Point in Time (PIT) Count calculates individuals experiencing homelessness twice a year and includes both people staying in shelters and staying unsheltered.

Homeless Management Information System (HMIS) is a community-based software system where service providers enter client data. The data is used to understand the scope and dimensions of homelessness while providing homeless clients and service providers a more efficient use of coordinated services.

Community Homelessness Center of Lancaster County (CHOC) connects individuals to basic hygiene needs and existing service providers. CHOC serves as a primary point of entry for chronically homeless people to gain access to the help they need.

Community Homeless Assessment & Referral Team (CHART), staffed by PA 2-1-1 East, is the single point of entry for individuals and families experiencing homelessness or are at imminent risk of becoming homeless in the next 30 days.

Examples of successful collective impact partnerships exist in Lancaster County. The Ten-Year Plan to End Homelessness is one that is tackling the plight of homeless families and adults in our community.

A challenge too big for one organization to solve, 12 organizations, government entities, and dedicated volunteers joined forces in 2008 with “Heading Home: The Ten-Year Action Plan to End Homelessness in Lancaster County.” The plan’s original major premise remains–guiding Lancaster’s service system from one that manages homelessness to one that focuses on prevention, diversion and rapid rehousing in order to end homelessness.

The power of this coalition produced results which attracted a $2.5 million federal grant. This was possible only when multiple players aligned to a common community goal. Today, they measure progress, communicate, and dedicate resources to this ongoing effort to reduce homelessness throughout Lancaster County.

United Way will invest in collaborative efforts that promote healthy living and behaviors and effectively integrate community prevention into health services delivery. These interventions will maximize enrollment in health insurance programs and provide access to medical, dental, and mental health care.

Community prevention improves the physical and social environment in which people live, work, and play. When individuals and families are connected to a medical home a team of providers works together to improve physical health, behavioral health, access to community-based services, and management of chronic conditions.
