Overview of Methodology and Results

In June of 2013, The Vision Council Research Department, delved into the VisionWatch database to determine how previous refractive surgery patients are using various forms of eyewear and vision correction. We also wanted to examine how the usage and buying habits of consumers who have had refractive surgery in the past varied from typical vision correction users who have not had any type of refractive surgery procedure. We examined the VisionWatch database for the year-ending period March 2013. There were a total of 118,591 adult VisionWatch survey takers during the 12 month ending period March 2013. Approximately 5,052 survey respondents (4.3% of the total sample) reported having had a refractive surgery procedure at some point in their lives. For the purposes of this study, we are defining refractive surgery as either a LASIK surgery procedure, a custom LASIK surgery procedure, or a Wavefront LASIK surgery procedure.

We filtered the series of reports and tables used to construct our standard VisionWatch reports to only examine the usage and buying habits for those 5,052 respondents who reportedly had a refractive surgery / LASIK procedure. We then developed an output file that compared the eyewear habits and behaviors of the refractive surgery patients to the eyewear habits and behaviors of the entire US adult population over the age of 18. The nineteen data tables in this report provide a topline comparison of all adult consumers (data highlighted in red in the subsequent tables) compared to just those who reportedly had a refractive surgery procedure at some point in the past. This is a follow-up report for a study first issued in 2011. In our Key Summary overview, comparisons of results from 2011 and 2013 are mentioned if they are noteworthy and crucial.

In addition to providing a topline comparison of all adult consumers vs. just refractive surgery patients, we were further able to breakdown the population of refractive surgery patients by gender (male and female), annual household income (Up to $60K per year and over $60K per year) and time elapsed since surgery (adults who have had a surgery procedure within the past year, adults who have had a surgery procedure 1 to 5 years ago, and adults who have had a surgery procedure more than 5 years ago). This additional level of detail provides important insight into how vision correction habits and behaviors change as time elapses since the consumer has had a refractive surgery procedure. We hope that members of The Vision Council will review the results of this report, and that it is helpful when developing long-term strategies for product development, marketing and customer outreach.

June 2013 Vision Council Report: Eyewear Habits of Refractive Surgery / LASIK Patients— Key Summary

Profile of refractive surgery / LASIK patients Of the 5,052 respondents who reported having a LASIK procedure at some point in their life, most were female (54.7%), most were under the age of 55 (77.3%) and most came from higher income households--$60K+ per year (58.2%). These demographics are different from the demographics of the general adult population of the US. Specifically, refractive surgery / LASIK patients are more likely to be female, younger and come from higher income households. In terms of time elapsed since having vision correction surgery, only 7.4% of our surgery patient respondents (372 adults) had their surgery procedure within the past year. Roughly 25.7% of our surgery patient respondents (1,298 adults) had their surgery procedure one to five years ago. The vast majority of our surgery patient sample (67.0% representing 3,382 adults) had their surgery procedure more than five years ago. Considering that refractive surgery / LASIK has been a viable vision correction alternative since 1996, it shouldn’t be a surprise that most of the surgery patients in our sample had the surgery more than five years ago. In total, we estimate that since 1996, there have been approximately 13.1 million American adults who have had a refractive surgery or LASIK procedure. When compared to our 2011 report, today we are seeing more female surgery patients and more surgery patients from higher income households. Moreover, when compared to the 2011 report, we now have fewer surgery patients who had a procedure one to five years ago. This is likely because of the steep drop n surgery procedures from 2008 through 2010.

Do refractive surgery / LASIK patients still need any type of vision correction? Yes, just under half of all refractive surgery / LASIK patients (6.5 million people) currently use some form of vision correction (vs. 76% of the total adult population in the US). Patients who recently had surgery were less likely to use some form of vision correction when compared to those who had their surgery procedure more than 5 years ago. As you might expect, the number of “squinters” who have had refractive surgery / LASIK is significantly lower than the number of “squinters” in the total adult population. A squinter is a person who does not use any form of vision correction even though they readily admit that they have a vision problem that impairs their sight. Approximately 530K refractive surgery / LASIK patients have some type of vision problem and do nothing to correct it. Most of these patients had their surgery more than 5 years ago and most are over the age of 55. Even though significant differences exist between the general population and the folks who have had a surgery procedure with regard to the presence of uncorrected vision problems, the two groups are very similar in terms of why they do not seek to correct their problems. Both groups claim that either they just “haven’t gotten around to fixing their vision problem yet” or that “their eye sight isn’t bad enough yet” to warrant a trip to the eye doctor.

What kinds of eyewear and vision correction do refractive surgery / LASIK patients use? As you might expect, refractive surgery / LASIK patients are far less likely than the general population to use various forms of prescription vision correction. In particular, only 25.7% of previous surgery patients use Rx eyeglasses, only 8.3% of previous surgery patients use contact lenses and only 5.7% of refractive surgery patients use Rx sunglasses. Women who have had refractive surgery / LASIK were slightly more likely than male surgery patients to use Rx eyeglasses or contact lenses to correct vision problems. Also, surgery patients who had their surgery more than five years ago were significantly more likely to use various forms of Rx vision correction when compared to adults who had their surgery more recently. It should be noted that even surgery patients who had their surgery more than five years ago, as a group, are still far less likely to use Rx vision correction when compared to the usage rates of the general population. When compared to our 2011 report, it seems surgery patients are now reporting less usage of Rx eyeglasses or contact lenses to correct any possible vision problems.

Interestingly, while Rx vision correction usage among surgery patients is far less than the national average, usage rates for plano sunglass and OTC reading glasses among refractive surgery / LASIK patients is far higher than the national average. Over 96% of refractive surgery / LASIK patients currently wear plano sunglasses (vs. 86.0% of the total adult population) and almost one-fifth of all surgery patients currently wear OTC reading glasses (vs. 11.8% of the total adult population). Again, as more time elapses from the date of the surgery procedure, usage rates for plano sunglasses and OTC reading glasses increases. When you look at high-end ($50+) plano sunglass usage, refractive surgery patients were 50% more likely to wear high-end premium plano sunglasses when compared to the general adult population. However, surgery patients were markedly less likely to wear clip-on sunglasses, probably because they are less likely to wear Rx eyeglasses and thus less likely to need clip-on sunglasses.

Do refractive surgery / LASIK patients have their eyes examined more frequently than the general population? Yes, those people who have had a refractive surgery procedure were slightly more likely to have had an eye exam within the past 6 months or the past 12 months. In fact, 43.8% of the total adult population has had an eye exam within the past year, while 49.5% of all refractive surgery patients have had their eyes examined within the past year. As you might expect, almost 100% of those who had their refractive surgery / LASIK procedure within the past year also had an eye exam within the past year. Frequency of exams decreases as more time elapses from the surgery procedure, suggesting that as time goes on many past surgery patients revert back to the eye exam habits and frequencies of the general adult population. It is also worth noting that eye exam activity and numbers are far higher among surgery patients who currently use some type of vision correction when compared to the exam activity of surgery patients who do not currently use some form of vision correction.

Do refractive surgery / LASIK patients buy eyeglasses? Yes, of the 69.3 million pairs of Rx eyeglasses purchased in the US during the 12ME period March 2013, about 551K pairs were purchased by consumers who had a refractive surgery / LASIK procedure in the past. In fact, it is estimated that 15% of the 3.4 million eyeglass wearers who had a surgery procedure in the past purchased Rx eyeglasses during the 12ME period March 2013. That indicates that surgery patients who wear Rx eyeglasses were far less likely to buy eyeglasses than other eyeglass wearers in the US. For all eyeglass wearers, the eyeglass re-purchase cycle was 2.2 years in March 2013. During this same time period, the re-purchase cycle for refractive surgery / LASIK patients who wear eyeglasses was 6.1 years.

As you might expect, women accounted for a larger share of eyeglass purchases among surgery patients, as did surgery patients over the age of 45 and surgery patients from higher income households. Moreover, a large majority of eyeglass purchases (80%) occurred among surgery patients who had refractive surgery / LASIK more than 5 years ago.

Most eyeglass purchases (86.9%) made by refractive surgery /LASIK patients were complete pair purchases. Surgery patients did make over 80K lens-only purchases during the 12ME period March 2013. While this figure represents only a small portion of all lens-only sales during the 12ME period March 2013, surgery patients were more likely than typical eyeglass wearers to purchase just a pair of lenses to use with an older pair of frames. There were also some rather noticeable differences in the lenses purchased by refractive surgery / LASIK patients when compared to other eyeglass buyers. A large majority of the lenses bought by surgery patients were single vision lenses (70.7%). Surgery patients were significantly less likely to buy multi-focal lenses or progressive-added lenses during the 12ME period March 2013 when compared to other lens buyers. However, when compared to all eyeglass wearers, lens buyers who have had a refractive surgery / LASIK procedure were more likely to have purchased lenses with photochromic coating and/or AR coating. Surgery patients were also more likely to have bought polarized lenses or Rx sunglasses when compared to the general eyeglass wearing population in the US.

Do refractive surgery / LASIK patients buy plano sunglasses or OTC readers? Yes, in addition to being more likely to wear plano sunglasses and OTC reading glasses, refractive surgery / LASIK patients were also more likely to have purchased plano sunglasses and/or OTC readers during the 12ME period March 2013. During this period of time, surgery patients bought 5.54 million pairs of plano sunglasses and 1.88 million pairs of OTC readers. Although not tremendously different, the re-purchase cycle for both plano sunglasses and OTC reading glasses was slightly lower for surgery patients indicating that those people who have had surgery generally purchase sunglasses and OTC readers with greater frequency when compared to consumers who

have not had a refractive surgery / LASIK procedure. Similar to what we observed with Rx eyeglasses, female surgery patients, as well as surgery patients from higher income households and surgery patients who had a procedure more than 5 years ago, were more likely than other surgery patients to have purchased plano sunglasses and/or OTC readers during the 12ME period March 2013.

Do refractive surgery / LASIK patients plan to buy eyewear in the future? Yes; however, the intended rates of purchase differ based on the specific product being examined. For Rx eyeglasses (complete pairs) and Rx lenses, purchase intent for surgery patients is considerably lower than it is for all other eyeglass wearers in the US. In March of 2013, just 4.6% of refractive surgery / LASIK patients were extremely likely to buy a pair of eyeglasses at some point in the future (vs. 9.4% of all eyeglass wearers in the US). Purchase intent was higher for female surgery patients and eyeglass wearers who had their surgery procedure more than 5 years ago. The situation for lens-only purchase intent was almost identical, with most of the anticipated lens-only purchases occurring among female surgery patients who had their surgery over 5 years ago.

When examining premium plano sunglass purchase intent, it seems as if surgery patients are more likely than the general population to be interested in buying a pair of high-end premium plano sunglasses at some point in the next 6 months. Just over 7.6% of recent surgery patients are extremely likely to buy a pair of high-end plano sunglasses within the next six months. Another 12.7% are very likely to make a high-end plano sunglass purchase. The corresponding numbers for all consumers are just 3.9% and 7.8% respectively. Unlike other products, purchase intent for high-end plano sunglasses (along with intent for eye exams) is higher among recent surgery patients when compared to consumers who had a surgery procedure more than one year ago. This is a sign that many new surgery patients are interested in protecting the recent investment they made in their eyes by getting an eye exam and wearing protective sunwear.
