Kenpo 5.0 Come learn martial arts and self-defense!
Who: For individuals with hearing impairments. The instructor knows ASL and can cater to individuals. Class is taught in ASL. You must know ASL to attend.

Indemnification & Refund Policy (Signature Required) 1.Release & Indemnification: I hereby, recognize and acknowledge that my/my child’s participation in recreational activi-
ties may involve bodily injury and/or emotional injury to myself and/or my child. In consideration of my/my child’s being
able to participate in such events, I, for myself/my child, my heirs, my executors and administrators, hereby voluntarily
and knowingly indemnify and hold harmless, defend, release, waive, and discharge Salt Lake County, and its officers,
employees and volunteers from any and all suits, claims or liability, including negligence, based on any injury except that
caused solely by the willful misconduct of Salt Lake County, that may result from my/my child’s participation in Salt Lake
County Parks & Recreation activities. In addition, I agree that my insurance company or I will pay for medical, hospitali-
zation or any other expenses resulting from my/my child’s participation.
2.Refund Policy: As per Salt Lake County policy and procedures, the Parks and Recreation Division may withhold %25 of
the refund (program registration fee) for administrative costs. All refunds must be requested in person and accompanied
with a written refund request. No refunds shall be given after the first day of the program.
3.Collections: I agree to pay Salt Lake County all costs incurred, together with reasonable attorney’s fees in the vent that
my account is referred to the Salt Lake County Attorney’s Office for collection. I understand that any account delinquent
30 days or more will be turned over to the Salt Lake county Attorney for collection.
4.Emergency Treatment: I hereby authorize Salt Lake County Parks & Recreation program staff to act on my behalf in
accordance with their best judgment in case of an emergency involving myself/my child, and agree to assume full respon-
sibility for all expenses, medical or otherwise, that may arise there from. I understand that my insurance company or I will
pay for such emergency treatment.

All Passes allow access for member and one free assistant to All County Fitness Centers.
Depending on type of pass gives you access to Indoor Pools, Ice Staking, Outdoor Pools and ALL County Fitness Centers
1. A statement signed by a licensed physician attesting that the
applicant has a permanent physical, mental, or sensory impairment
that severely limits one or more major life activity, and states the
nature of the impairment. 2. A document issued by a federal agency, such as the Veteran's
Administration, which attests that the applicant has been medically
determined to be eligible to receive federal benefits as a result of a
disability. Other acceptable federal agency documents include proof of
receipt of Social Security Disability Income (SSDI) or Supplemental
Security Income (SSI) due to a disability. 3. A document issued by a state agency, such as the vocational
rehabilitation agency, which attests that the applicant has been med-
ically determined to be eligible to receive that agencies benefits or ser-
vices as a result of medically determined permanent disability. Show-
ing a state motor vehicle department disability sticker, license plate, or
hang tag is not acceptable documentation.
To receive this benefit make an appointment and submit the
documentation listed above in person to Salt Lake County Adaptive
located at the Copperview Recreation Center, 801-559-1500.
A Salt Lake County resident, who is permanently disabled,
and can provide one of the following pieces of
documentation to receive a facility pass that permits the
holder an assistant entrance fee at no cost.

Summer Camp
Application Deadline: Friday, May 14th Come in or Call Redwood for an Application. Application Fee is $15.
Raiders This camp will also have lab time, go on field trips once a week, participate in arts and craft, sports and other activities. This will be a strict camp with rules and regulations. We will be asking for parent involvement to work on goals and behavior modifications for the success of all students in this program.
Who: For ages 5-12 years old with cognitive disabilities and mild to moderate behavior disorders that can function in a 4 to 1 ratio.

Trailblazers This camp will be focused on socialization and fun! They will be involved in arts and crafts, some sports and lots of educational learning. There will be swimming once a week along with time in a computer lab and a field trip.
Who: For ages 13-21 years old with cognitive disabilities that can function in a 7 to 1 ratio.

Special Olympic Training Programs
Adult Basketball League For adults 18 years and older that want to play on a team and practice their basketball skills. This is for beginner to advanced skill levels.

Summer Camp

Pathfinders Kids and teens with cognitive disabilities or dual diagnosis of physical and cognitive disabilities. In this group participants that need some minor assistance with physical needs of Daily Living or enjoy a slower pace of fun activities. Participants will be involved in arts and crafts, field trips, wheelchair sports, and educational learning. No personal Care is provided
Who: For ages 5-21 years old with physical disabilities that can function in a 7 to 1 ratio.

Pirates This camp is sports orientated for those that like to move and interact with others. They will learn new skills and movements in sports including; basketball, soccer, softball, flag football and more. The camp will go to Mick Riley Golf Course once a week. They will be certified in Special Olympic golf by the end of camp. This camp will also have comput-er lab time, go on field trips once a week, participate in crafts and other activities. This camp is a great fit for anyone that loves competing in the Special Olympics.
Who: For ages 10-21 years old with cognitive disabilities that can function in a 7 to 1 ratio.

League Bowling Practice your bowling skills while socializing with friends. The session lasts 8 weeks held on Thursdays. Each week includes up to two games and shoes. Who: Individuals 8 years and older. 1 to 10 ratio. Participant can bring someone to assist them. This is a league that will get you ready for Special Olympics . Saturday Socials
This is a social club that allows adults with mild to moderate
disabilities to experience socialization and recreation in an age-
appropriate environment with their peers. Individuals will have the
opportunity to participate in a wide variety of monthly programs and
community integration activities such as dances, sports, athletic
activities, art, theatre and museum trips, also volunteer opportunities
and more.

Adult Day Program A structured day recreation program for adults with mild to moderate disabilities. Activities include fitness, computers, outings, leisure education, planning, games, socialization, FUN and more! Participants bring their own lunch each day. No personal care provided. Fees must be pre-paid for participation.
When: Monday thru Friday except major holidays.
Time: 11:00 a.m.– 3:00 p.m.

Cost: $13 per day $130 per month (Monday, Wednesday, Friday) $230 per month (5 days a week)
Socialization Programs
Saturday Night Dance A live DJ spins popular favorites at our bi-monthly dances for individuals with disabilities ages 16 years and older.
When: Saturday evenings: March 5, 19 June 4, 18 April 9, 23 August 20 (Luau) May 7 (Cinco de Mayo) May 21
Time: 6:00 pm – 9:00 pm Doors open at 6:00 pm
Where: Copperview Recreation Center, 8446 S. Harrison St.
Cost: $5.00 paid at the door, $3 dollars is admittance to the dance and $2 goes toward playing the Wii, mini golf, karaoke or food at the “Café on the Green”. **Support Staff/Chaperones : FREE entrance to the dance
