He has published numerous articles and chapters on dialectical behavior therapy (DBT), trains professionals and students in DBT, and has coauthored books on behavior therapy, borderline personality disorder, and self-harm. In 2011, he received a Michael Smith Foundation for Health Research Career Investigator Award to support his work on borderline personality disorder, as well as a Canadian Psychological Association Early Career Scientist Practitioner Award for his work integrating research and treatment in DBT.

Dialectical behavior theraPy (Dbt) has brought hoPe For recovery to countless PeoPle suFFering FroM coMPlex Mental health ProbleMs. Before DBT poured out of Marsha Linehan’s research laboratory about 20 years ago, getting help for borderline personality disorder (BPD) and related problems was much like wandering the desert looking for water. Sometimes, water was available, but it was almost never enough, and at other times, those who provided the water were inhospitable.

DBT became one of the first consistent and reliable water sources when Linehan published results from her first randomized trial of DBT (the first published randomized trial of any treatment specifi-cally for BPD). Moreover, the tremendous enthusiasm with which this compassionate, practical, and non-judgmental treatment has been embraced and adopted around the world has softened some of the hostile terrain in which our patients used to wander. As a result, DBT has not only instilled hope, but it also has brought a message of compassion that, I believe, has been chipping away at some of the stigma surrounding those with BPD and related problems.

Interestingly, when Linehan was developing DBT, she had actually set out to develop a treatment that would work for highly suicidal women, not necessarily for BPD. As she tried to apply standard cognitive behavioral therapy (CBT) with these women, many of whom had BPD, she learned from her successes and failures that effective treatment required a sophisticated balance of acceptance and change. She then used the best available science, as well as the wisdom of her clinical experiences with these women, to piece together a treatment that was not only palatable but also very effective. Not only did therapists need to accept and understand their patients, but the patients needed to learn how to accept themselves, their thoughts, emotions, situations, and others. DBT became one of the first therapies to systematically help clients learn how to live in the present, become more mindful, and accept reality

exactly as it is. At the same time, DBT employs the best of behav-ioral science to not only keep patients alive, but also to develop lives worth living. In this way, patients learn not only to find water, but also to build liveable homes.

Although the research evidence clearly shows that DBT works (there are over a dozen randomized clinical trials, numerous additional tri-als and studies), over the past several years, we have begun to learn a lot about how and why DBT works and to expand the horizons of DBT beyond standard outpatient therapy for persons with BPD. For example, adaptations of DBT have emerged for suicidal adolescents (with a large, multi-site trial underway as we speak), older adults with depression, suicidal college students, patients with bipolar disorder, and prisoners in the correctional system, to name just a few. Moreover, studies are beginning to investigate mechanisms of change in DBT (including some exciting work on brain changes through DBT); we are starting to learn whether and when the whole treatment (individual therapy, skills training) is necessary and when we can use a trimmed down treatment package; and we are discov-ering what predicts successful outcomes in DBT.

As this work continues, it will be increasingly important to dissemi-nate DBT theory, research, and practice to front line clinicians. Behavioral Tech, LLC. This special section on DBT also represents New Harbinger’s strong commitment to the dissemination of DBT resources. New Harbinger has made some big strides in this area, publishing several practical guides to different aspects of DBT for clinicians and for clients struggling with a variety of difficulties, such as anxiety disorders, bipolar disorder, eating disorders, among others.
