2012 - 2013 REGISTRATION FORM

MARCHING BAND – COLOR GUARD

Welcome to the Michigan City High School Wolfpack Band Program! As part of the Michigan City High School Wolfpack Marching Band, each student will receive a Marching Band t-shirt to be worn as the official “Wolfpack Band tee-shirt” under the uniforms. Students will be required to wear these shirts for Marching Band activities. Please be sure to include the student’s size on the Registration Form. The Boosters email band news and important information, forms, calendars, and much, more! Make sure your email address is listed on the registration form and visit with us at [email protected]. A Registration Form is required for all students who are participating in the Michigan City Wolfpack Marching Band. A $25.00 non-refundable deposit for Marching Band is due by April 1, 2012. This deposit will go towards your Marching Band payment (leaving a remaining balance of $75.00 due on or before the first day of Band Camp). This deposit holds your place in the show. No late registration or deposits will be accepted, as I have to give the final number of participating students to the Drill/Music designer. Please complete all of the information on the registration form, attach a check, place it in

an envelope with the student’s name clearly labeled and return it to your Middle School Band Director, the MCHS Band Director or by mail to Mr. Quincy Ford, Michigan City High School, One Wolf Trail, Michigan City, IN 46360. Checks should be made payable to the MCHS Marching Band. Please place your student’s name and the reason for the payment on the check. All fees must be current for your student to participate in activities. All payments must be paid or other arrangements must be made prior to the start of Band Camp in order for the student to participate. Any questions regarding fees may be directed to Mr. Quincy Ford. Payment Schedule: Michigan City High School Wolfpack Marching Band $25.00 Non-Refundable Deposit Due by April 1, 2012 (if amount is already paid, please note on the form) $75.00 Remaining balance towards Marching Band due on or before Band Camp.

MCHS Marching Band Uniform Agreement

All Members of the Marching Band will be issued the following when joining:

1. Summer Uniform a. Grey T-Shirt with Marching Band Logo b. Navy Shorts

2. Performance Uniform a. Jacket b. Navy Pants c. White Pants d. Helmet e. White Gloves f. Marching Shoes

All members of the Color Guard will be issued the following when joining:

1. Summer Uniform a. Grey T-Shirt with Marching Band logo b. Navy Shorts

2. Performance Uniform a. Appropriate costume for performances b. Dance Shoes

As a member of the Marching Band or Color Guard, it is the responsibility of the student to keep their uniform in performance quality. Uniforms that require dry cleaning are done so BEFORE they are issued to the member.

No member is allowed to eat or drink anything while in uniform, except for water unless the Band Director gives expressed permission.

Should any uniform need cleaning before the end of the season, due to misuse by the band member it will be the responsibility of the member and his/her parents to have the uniform cleaned at their expense.

Should a member misplace/loose damage or request a new uniform item, there will be an additional charge to replace said item. A replacement list and charges will be available upon request.

MCAS Policies on Student Conduct & Discipline Acknowledgement

The Michigan City Area Schools policies on Student Conduct and Discipline shall be in full

force and effect as to all student participants in this function/trip at all times during the same, and any violation of any rule(s) contained therein by our child may result in appropriate disciplinary measures including suspension and expulsion as provided in said policies. Responsibilities It is a privilege to participate in and be associated with MCHS Marching Band. This privilege is extended to all band students, provided they are willing to assume certain responsibilities. A student must be willing to make necessary sacrifices in order to be a credit to himself/herself. This can be done by: A. achieving academically by first being a good student. B. exhibiting high standards of social behavior. C. displaying sportsmanship, both on and off the playing field. D. respecting other bands, officials, spectators, and those in authority. E. being cooperative. F. maintaining a good appearance, including cleanliness and good grooming. G. using language that reflects well on family, school and self. H being a positive leader by example, words and/or actions. I. complying with the rules necessary to be in good standing at the completion of the marching season (the last contest or the banquet, whichever comes later). Eligibility* As outlined by the Michigan City Area Schools, the student must: A. be in good standing with the school; B. have earned passing grades in five (5) or more full credit subjects during the previous grading period or semester, and be currently enrolled in and passing five (5) or more full credit subjects; C. have a minimum grade point average of 1.5 on a 4.0 scale for the previous grading period or semester. The following general rules for participants have been established: A. A participant shall not violate local and state laws, and the Michigan City High School’s “Code of Responsible Behavior”. B. Abuse of school owned equipment is prohibited. C. A participant must attend a full day of school the day in which he/she participates in competition, performance, or practice unless excused by a Principal or Director. D. A participant shall not possess or use tobacco products. E. A participant shall not consume or be in possession of alcoholic beverages. F. A participant shall not knowingly possess, use, transmit or be under the influence of any narcotic drug, hallucinogenic drug, stimulant, controlled substance, or possess, use or transmit paraphernalia for use of such substances. G. A participant is discouraged from being in attendance at a place where he/she knows that illegal consumption or possession of alcohol is occurring, and/or transmission or use of substances listed in Section D-6 of the student handbook. Attendance at these events may result in ineligibility. H. A participant shall not use or possess weapons. Dress Code While the administration recognizes the individuality of each student and does not wish to stifle individual taste, consideration must be given to the primary purpose for being in Marching Band. Clothing and/or personal adornment that is unusual, unsafe, or attracts undue attention and distracts from the academic climate of the classroom or school function setting shall be considered inappropriate. Dress code infractions fall into five (5) general categories:

• Health/safety factors • Clothes that are too revealing • Items that are a disruption/distraction from the educational process • Items promoting profanity, lewd, or inappropriate language • Items promoting illegal activity

I/We agree that our student will attend all required practices, camps, performances, and we will pay all fees associated with the activities for which we have registered. I/We understand the contact information on the Registration Form may be used by the Band Directors and Music Boosters to give important information to parents and students. I/We agree to the terms of the MCHS Marching Band Uniform Agreement found on a separate page in this packet.
