Admission to the Hegau-Jugendwerk depends on the type and the date of occurrence of

the patient’s injury or onset of illness, his/her current condition, and the special goals to be achieved in the respective rehabilitation units. We offer a full spectrum of diagnostic and ther-apeutic services. This allows us to ensure that our patients receive comprehensive and continu- ous rehabilitative care, from the time they are dis- charged from hospitals providing acute patient care up until they are reintegrated into community education, occupational and social areas of life.

Thus, the important features of the Rehabilita-tion Centre include the on-site, state-accredited school (Wilhelm-Bläsig-Schule) and the depart-ment for occupational and vocational therapy.

We admit children, adolescents and young adults. Patients requiring daytime care are also admitted.

the rehabilitation concept

Individually tailored for each patient

Page 7: 2011 - Hegau-Jugendwerk Gailingen GmbH · 2011 Neurological hospital and rehabilitation centre for children, adolescents and young adults. Hegau-Jugendw erk ... special health problems

Upon admission to the Hegau-Jugendwerk, an individual therapy plan is developed for each patient. This plan is tailored to address the patient’s

special health problems and individual therapeutic needs and goals. Such goals may include improving or regaining physical and cognitive abilities, attaining independence in self-care at home and dealing with activities of daily living. Other equally important goals for many of our patients include becoming reintegrated in school life, continuing or pursuing vocational training or returning to the workforce. In order to achieve these goals, we follow a systematic, progressive approach in de-veloping the therapy plan for each patient.

An interdisciplinary team of experienced rehabilitation professionals works in collaboration with and under the supervision of specialist phy-sicians in our hospital to develop and oversee this plan throughout the entire rehabilitative process.

In regularly scheduled team conferences (”case management” meetings), the team members exchange information about the patient’s progress – also in co-operation with the patient and his or her family – and review the content, scope and goals of the therapeutic plan. The team may revise the plan so that the patient receives all necessary medical treatments, therapies and optimal educational and occupational training.

criteria for admission

Comatose/apallic patients (without increased brain pressure and stable vital functions) and patients who are in the early stages of remission

following brain damage and still require intensive medical monitoring and treat-ment are admitted to the Rehabilitation Unit for Intensive Care (Frühreha- bilitation). We also treat patients requiring respiratory assistance.

When patients no longer require intensive medical care and are able to actively participate in therapies for at least short periods of time, they may be admitted to the Rehabilitation Unit for Severely Disabled Patients (Schwerrehabilita- tion). In addition, we admit patients with less recent injuries but whose resulting dis-abilities necessitate intensive medical and therapeutic care.

The General Rehabilitation Unit (Allgemeine/Anschlussrehabilitation) admits patients who are largely self-sufficient in carrying out most everyday activities. Wheelchair-bound patients are also admitted.

Patients requiring vocational rehabilitation should be physically and cognitively able to participate in the training programs.

The setting in the Children’s House offers therapeutic and nursing services for children who require an increased amount of care and supervision due to physical and cognitive impairments.
