Research Funding:So Many Projects, So Li� le Money!

The joy and the missteps of fi nancialrecovery is not much fun for all of us,especially when it comes to researchfunding, which seems to have taken abackseat to other concerns during thisfi nancial recession.

Yet there are many outstanding projectsthat have been submi� ed to your Foundationpromising to add value to physiatric care,and we just don’t have enough money to sustainthem all. For every research grant that we give out,there are two to three additional projects thatmerit support that we can’t provide. Everyone is beingpressured; everyone is being squeezed. But the future of our fi eld is dependent on the researchof today. Once again, I ask you to dig a li� le deeper and continue to support your Foundation.

Your donated money is used to support valuable research projects. As you know, these projectshelp improve the lives of the people that we serve, and provide outcomes and eff ectivenessdata that is increasingly relied upon for reimbursement. The information acquired from suchresearch will substantiate that your clinical practice maintains its’ cost eff ectiveness and clinicalcompetence.

5K BONE RUN/WALK

& ROLL

The major fundraising event for 2009took place on Friday, October 23 in Austin, TX..Thank you to the nearly 200 runners andwalkers who showed up before dawn forthe Foundation for PM&R’s Rehab 5KRun/Walk & Roll.. This event raisedapproximately $15,000 for physiatricoutcomes research.

Drs. Christopher Hess (Charlo� esville,VA), Geoff rey Keenan (Charlo� esville,VA) and Mederic Hall (Rochester, MN)took top honors, coming in at 16:52:15,17:52:92 and 18:10:42 respectively.Dr. Jennifer Clark (Columbia, MO) was thefastest female runner at 21:50:71, whileDr. Tom Pierce (Quantico, VA) was the winnerin the 60+ category at an impressive 22:35:04.

The AAPM&R Resident Physicians Council (RPC) supported the event by holding a challengeamong all physiatric residency programs. The University of Virginia at Charlo� esville sweptthe competition, winning the top prizes for both Fastest Runners and Most Entrants. The RPCBoard also provided volunteers to assist with onsite registration.

The Foundation for PM&R deeply appreciates the support of our corporate sponsors: CasaColina Rehabilitation Center, St. David’s Rehabilitation Hospital and Morgan Stanley-The TateGroup. Special thanks, too, to Dr. Charlote Smith and SETON Brain and Spine Center forproviding the emergency medical response team; Bruce Wilson of South Jersey Health Systemfor invaluable assistance in managing and onsite coordination of the event; and Drs. GeorgeKevorkian and Joseph Volpe for serving as local hosts.

The Research Capacity Building Initiative wasestablished to meet the needs identifi ed at the2005 Research Summit and to serve the fi eldby identifying ways to expand physiatricresearch facilities. The Foundationappointed a Research Summit Follow-upTask Force, chaired by Dr. WalterFrontera, to review the recommend-ations of the Summit and puttogether plans for implementation.

Dr. Frontera and the Task Force enlistedDrs. Leighton Chan and MichaelBoninger as lead authors on a two-partwhite paper titled “Best Practices forBuilding a Successful Physiatric ResearchCenter in an Academic Medical Center”(Am JPhys Med Rehab 2009: 88; 659-66). According toDr. Chan: “These papers outline challenges thatdepartments face in expanding rehabilitation research and highlight how successful programsovercome these barriers.”

The contents of the papers, including case studies, were developed into a two-part workshopoff ered at the 2009 AAPM&R Annual Assembly in Austin, TX and in condensed format at theResearch Council Meeting during the 2010 AAP Annual Meeting. The Foundation and theAcademy collaborated to provide the contents of this educational program free of charge onthe AcadeME site in two slide lecture sessions.

RESEARCH CAPACITY BUILDINGINITIATIVE

In 2007, the Foundation conducted a study to evaluate the admissions criteria included in the local coverage determination (LCDs) of Medicare fi scal intermediaries. Using the Standards created by the American Academy of Physical Medicine and Rehabilitation as a measure, the results found 5 of 10 LCDs (50%) did not meet the Academy’s standards.

The results of this study generated in-terest in coverage denials, particularly those cases submi� ed for appeal to an Administratve Law Judge (ALJ). The Foundation assembled a team of experts who met in Washington, DC in July 2008 to analyze the factors leading to the ALJ decision. From the 70+ cases submi� ed, 24 cases were selected to represent a broad range of diagnoses and geographic diversity. The Academy’s standards were used as a benchmark to analyze these ALJ decisions in an a� empt to gain insight into the qualities of the standards, as well as an under-standing of the case denial process. The team examined a sample of 24 IRH/U admissions to determine what factors contributed to the Medicare coverage denials, and to compare the determinations with professional standards and expert medical opinion. All cases were denied by the Fiscal Intermediary and on appeal to the Qualifi ed Independent Contractor then subsequently appealed to an Administrative Law Judge. Experts achieved consensus in 21 of the cases that the decisions of the FIs and QICs did not meet professional standards. The expert review panel disagreed with the ALJ in 5 of 8 cases.

An article is being prepared for publication, and results were presented in poster format at the American Academy of Physical Medicine and Rehabilitation’s 2009 Annual Assembly in Austin, TX.

THE EDUCATION RESEARCH FUND (ERF)Since 2002, the Foundation for Physical Medicine and Rehabilitation has managed and adminis-tered the awards program of the Education Research Fund. In early 2010, the fund was renamed the Richard Materson ERF Fund, in memory of one of its founders. The goal of the fund is to promote outstanding research in the fi eld of rehabilitation and recognize and encourage promising young researchers.

JUSTUS LEHMANN FUND Justus F. Lehmann MD (February 27, 1921 - April 28, 2006) served as the founding chairperson of the Department of Rehabilitation Medicine, University of Washington School of Medicine from 1957 to 1986 and trained many leading physiatrists. In memory of his accomplishments the Foundation Board established a Lehmann Fund to provide an education/research grant or award.

OSTEOPOROSIS FUND In 2008, the Foundation conducted a campaign to raise monies for research into osteoporosis and osteoporosis-related disabilities. The campaign included a 5K Rehab Bone Run/Walk & Roll fundraising event. Contributions continue to come in throughout 2009 to support osteoporosis-related rehabilitation research.

The KRUSEN CLUB (cumulative gi� s of $25,000 or more) was named for Dr. Frank Krusen, considered the founder of physiatry.
