Crane topple due to over weight load, unstable soil , side or front lifting, out riggers not fully extended & blocking not completely support the loads. 4 - Blocking under outrigger floats shall in such thickness, width, and length as to completely supports the loads, transmit under to the supporting surface, and prevent shifting, toppling or excessive settlement under load. Use of blocking only under the outer bearing surface of the extended outrigger beam. 2.3 Crawler crane 150 ton - at jetty 2.4 Crawler crane 35 ton - on barge 4 - Side loading & no loads shall be lifted over the front area, except as specified by the crane manufacturer. Damage to over head structure, underground services & coalition with other machinery in the vicinity. 5 - To stop operation in bad weather etc heavy rain & in excessive or strong winds. Inspecting of work site, trained signalman, lighting during night operation & to work at safe distance from other machinery. Crane hooks with deformation or cracks. Sling shackle failure, faulty rigging equipments. Deterioration or leakage in air or hydraulic system 5 - Frequent inspection from daily to monthly interval to all sling, wire ropes, chain, shackles, spreader beam, lifting lugs/pad eyes & operators log book etc. 4 - Crane must be fitted with a load radius indicator, automatic safe load indicator, reversing alarm & crane hook with safety latch. Before an excavator is used for lifting, it is important that the site management team have conducted the necessary assessments. Thought should be given to:

Provide designated storage area for waste & unused materials. Government or local Authorities

and with full compliances with Department of Environment (DOE) Act & regulations. EQ (Scheduled Waste) Regulations 2005, EQ (Sewage & Industrial Effluents Regulation 1979, Malaysian Air Ambient Guideline.

Segregation as per categories Domestic Waste / construction waste or Schedule Waste.

Engage separate contractor for disposal of waste material if necessary.

Special attention to areas reserved for waste having particular regard to its safe containment.

Treatment, storage, transportation, and disposal must be in accordance with applicable federal, state/provincial and local regulations. If approved, may be transferred to a land disposal site. If the spent grit contains contaminates at levels above those specified under DOE, then the waste is defined as hazardous and must be managed per federal or state regulations governing hazardous waste.

Remove and disposal in order to ensure that local population are not exposed to unnecessary risk.

Suitable covered receptacles shall be provided for the disposal all food wastes.
