Pre-screeningeligibility requirements:

• You are 18 years of age or older • Opiates are your primary drug

• You have been addicted to opiates for at least 12 consecu�ve months

And/or:• Currently pregnant

• Recently released from prison or incarcera�on and feel a risk for relapse• Previously treated pa�ents up to 2 years a�er discharge

Alliance Wellness Clinic, Inc. believes in treating all components of an individual’s

social, physical, spiritual, cultural and emotional well-being. We also believe that

dependence on opiates is medically treatable with proven, clinically

researched methods.

At Alliance Wellness Clinic, you will be accepted as an equal and not judged for your individual choices. We understand

that chemical dependency is a chronic and progressive disease with multiple causes

and we dedicate ourselves to o�ering specialized care, empowering people to create healing, meaningful change for

themselves

SUBOXONE & METHADONE Alliance Wellness Clinic, Inc. o�ers both Suboxone and/or Methadone medication to

help with opiate addiction. Both medications are used to help alleviate withdrawal symptomsand to reduce or eliminate cravings for opiates. Typically, the program lasts from 1-2 years,

depending on the individual needs of the patient. Licensed sta�, including nurses and medical doctors are available to provide treatment that will result in the most

positive outcome for the patient.

INDIVIDUAL COUNSELINGWith both forms of medication assisted treatment, patients will be o�ered individual

counseling sessions with an LADC (Licensed Alcohol and Drug Counselor). Sessions areo�ered to help with continued support throughout the medication treatment process. Patients

can be referred for individual Mental Health counseling as well as Outpatient Treatment,if they’re in need of more support.

ASSESSMENTSAlliance Wellness Clinic o�ers full chemical health assessments for patients that are in need of an

assessment for personal, funding, court, or etc. The assessment will typically be completed in session with a patient’s chemical health counselor. The assessment will inquire about an

individual physical, social, and emotional well-being as well as current living situationas to assess the best route for treatment planning.
