S3: UNEARTHING HUMANITY: AN INTERACTIVE DEMONSTRATION ON LGBT BIASES: PART 1Lynnette Lammers, MA | Office for Inclusion and Intercultural Initiatives, Michigan State University

Using an experiential theatrical framework that engages participants in collaborative problem-solving, the life of Aggie, an older LGBT adult, who is navigating a potentially terminal illness and end-of-life decisions will be explored. The diminished health outcomes for older LGBT adults, looking for remedies that range from personal to institutional to legal will also be examined. This interactive demonstration on biases towards older LGBT adults will provide participants with a better understanding and sensitivity of this unique resident population to promote enhanced quality and compassionate care.

Rehospitalizations present challenges to the facilities and hardships for the residents and families. Expectations and understanding about end of life care and the disease process can improve the quality of care provided by the facility and reduce unnecessary hospitalizations. Understanding how to start the conversation about advance care planning and how the collaboration and support of the whole interdisciplinary care team is crucial, can help improve resident satisfaction and reduce transfer to a hospital for services and symptoms that can be treated in the facility.

One of the major challenges facing health care providers is how to best handle an incident. Are you and your staff prepared? Incidents effect or impact more than just the resident but also the facility and staff. These concerns may include ramifications with the State, a potential lawsuit, or even a criminal investigation. The best prepared facilities have procedures in place so that staff know exactly what actions they must take and what documentation must be retained in the best interest of the resident involved but also to protect the facility and staff.

S6: LESSONS LEARNED IN LTC EMERGENCY PREPAREDNESS: IRENE VS. SANDYDavid Weidner, MPH, BS, REHS, MEP, CEM | Health Care Association of New Jersey

Elderly individuals suffer increased vulnerability to disasters due to impaired physical mobility, diminished sensory awareness, and pre-existing health conditions. These individuals often have extremely limited capacity to function and cope with even minimal stress and often suffer higher mortality rates than the general population during disaster events. Long-term care facilities are responsible for the care of many of these at-risk frail elders who are especially vulnerable to catastrophic public health events, yet have not always been adequately incorporated into the emergency management process. Yet, even after New Jersey was so adversely affected by Hurricanes Sandy and Irene, it is often difficult to drive home the importance of emergency preparedness to the wide array of hazards our operations and (subsequently the individuals that our members are tasked with protecting) face each year, whether natural or man-made. This presentation will compare HCANJ’s lessons learned from two hurricanes that impacted New Jersey.

This session will focus on the CMS 5 Star Rating system and its impact both as a public relations tool and internal use to improve performance and quality of life for the residents we serve. The three elements that constitute the 5 Star Rating will be reviewed with concentration on the Quality Measures impact. A review of all QMs and in particular those that impact the rating system will be outlined. Specific methods of interdisciplinary collaboration to improve the QMs and the overall 5 Star rating will be provided.

S9: THE RISKS OF SIDERAILS & ASSISTIVE DEVICESDavid Wiegel, JD, BS | The Kitch FirmJohn Hessburg, JD | The Kitch Firm

While thought to be a protective measure, bed siderails and other assistive devices can pose serious risks to residents. Facilities that use assistive devices must put in place, appropriate measures to protect their residents as well as their staff and facility. Additionally, alternate assistive device options have popped up in the industry, each trying to claim that they are not bed rails and do not carry the same risks. Providers must be aware of the risks these devices and bed siderails pose.

S10: MANAGING AGGRESSIVE/VIOLENT RESIDENT BEHAVIORS Michael Goldsmith, LMSW | Behavioral Care Solutions LLC

This presentation will provide long-term care providers with an overview on how to effectively care for residents who display aggressive and/or violent behaviors. This program will present the causes of behavior, assessment to determine appropriate plan of action, safety awareness and interventions. The content of this program can be implemented with success in many residential settings including adult foster care/group homes, assisted living communities and skilled nursing facilities.

S11: ANTIDISCRIMINATION: A LEGAL PERSPECTIVE OF THE LGBT EMPLOYEE & RESIDENTPatricia Nemeth, JD, LLM, BS | Nemeth Law PCTerry Bonnette, MS, JD, BS | Nemeth Law PC

Sexual orientation is not a protected class under either federal or state law. Nevertheless, the Equal Employment Opportunity Commission (EEOC) has announced as part of its Strategic Enforcement Plan, that it will address employment discrimination against LGBT individuals. Moreover, LGBT residents may be entitled to similar protections. This session will address LGBT rights of both employees and residents and how courts and administrative agencies are interpreting existing statutes to extend protections to LGBT individuals.

S12: INNOVATIVE PROGRAMMING FOR INTERDISCIPLINARY TEAM COLLABORATION Cathie Coleman, LNHA, ACC, RAC-CTDiane Zide, MS, BS, CTRS, ACC, LNHA | Botsford Continuing Care Corporation

This presentation will focus on innovative means to engage residents in meaningful activities with an Interdisciplinary team collaboration approach. Focusing on assessment and the MDS process (Resident Assessment Instrument) this presentation will present various means of providing activities in keeping with the resident interest and not focusing solely on the content of an activity calendar but more about engaging the resident in empowerment, maintenance and supportive activities that promote the highest degree of independence possible while continuing their previously enjoyed leisure activities.

VIRTUAL DEMENTIA TOURAt the 64th AHCA|NCAL Annual Convention & Expo in Phoenix, some members were asked to do everyday activities like pick up a glass of water and drink, but they did it with blurred vision, diminished hearing, and a desensitized grip.

These members were experiencing a sudden onset of diminished capacity by wearing tinted and sight-distorting goggles, earphones played disorienting noise, gloves with rubber stops in the palm, and spiky shoe inserts; all to simulate what an individual with dementia or Alzheimer’s disease experiences on a regular basis.

Voluntarily wearing this equipment simulates the loss of hearing and increased confusion, the loss of central and peripheral vision due to muscular degeneration, and the diminishment of sensory and fine motor skills frequently experienced by aging individuals.

The Virtual Dementia Tour, developed by Second Wind Dreams, a nonprofit organization based in Marietta, Ga., is a scientifically proven method of dementia care training that increases awareness and sensitivity in staff members. A kit combines educational resources with a simulation tool kit that enables individuals to experience the physical and mental challenges facing those who have some form of dementia or Alzheimer’s disease.

The setting has evolved from the monthly general practitioner visits for his own patients in the nursing home to Geriatricians and corporate entities of specialists (SNF-ists) in long-term care. Attracting providers to the facility can be a struggle; providing support to the provider is paramount to retaining the services. This presentation will address, from the provider point-of-view, the needs of the provider to provide excellent care with efficiency, how electronic medical records are utilized by the provider, and how Meaningful Use impacts providers regarding documentation and requirements.

This presentation address the process of running employee background checks for AFC/HFA and other assisted living settings and will provide participants with information on how to conduct a background check from start to finish. CMS and long-term care providers have never considered sleep as an integral part of the plan of care and services provided for the resident. This Restorative Sleep Vitality Program is a combination of nationally recognized evidence-based, sleep hygiene research studies and the application of cutting edge practices to enhance residents’ sleep & awake states. Providers have come to the forefront in recognizing that a good night’s sleep is one of the most important gifts we can give our residents. Sleep is not just a state we fall into, but a necessary and vital part of keeping residents healthy. With sleep in mind, new practices and standards of care have made adjustments to provide uninterrupted sleep at night and promote residents being awake and engaged during the day.

Our goal is to provide the best possible care for our residents. A growing concern has been the continued use of personal alarms that attach to or are placed next to or near the body of the resident. We find this practice to be intrusive and undignified to the quality of life of our residents. We strive to maintain the safest environment possible, but the use of personal alarms has not proven to be of assistance in meeting this goal. Alarms have been shown to have many negative effects on the lives of the residents. Alarms contribute to the immobility, restrictiveness, discomfort, restlessness, agitation, sleep disturbance, skin breakdown and incontinence of the resident. In light of this recent evidence and to improve the lives of our residents, we will conduct a systematic and careful assessment and evaluation for the successful removal and reduction of personal alarms in our care centers. In addition to the harm alarms cause by immobilizing residents, and having no evidence that they prevent falls or injuries, they are difficult to utilize in a consistent way.

Since 2011, she has served as Commissioner of the Vermont Department of Disabilities, Aging and Independent Living. A popular and nationally recognized speaker and advocate, Susan has educated students, policy makers, caregivers, administrators, physicians and other health and human service professionals from Alaska to Florida. Human diversity still very much affects the world in which we live and work. In particular, both residents and staff working within a long-term care environment often face racist and ethnocentric behavior, acts, and speech perpetrated both by other residents and staff members. This session will discuss ways to support residents’ rights, and support staff and resident safety and well-being, in racist and ethnocentric situations. In particular, strategies to diffuse or redirect behaviors, strategies to support the people experiencing racism and/or ethnocentrism, and offer ways to mitigate the impact of the behavior for the people affected will be discussed. Ways in which health care providers can promote a sensitive and tolerant learning culture in order to create a safe and inclusive environment for all will be shared.

This session will present an overview of NCAL’s resource on managing complaints and grievances. Assisted living communities/settings and skilled nursing facilities receive their fair share of resident and family complaints about many issues, including dining services, provision of care and more serious concerns such as theft of resident property or mistreatment of residents. The manner in which the provider receives and manages those complaints is critical to affecting the outcome and resolution of complaints. A comprehensive complaint and grievance process will be reviewed to help providers train their staff in the most effective conflict resolution techniques and how to view complaints as an opportunity for quality improvement.

The effects of dementia are based individually. While dementia progresses at a rate specific to each person, the question is how to best meet a person’s individual needs throughout their journey. Intimacy, touch, gratification, and partnership still exist within the confines of illness. This session will explore the complex effects of intimacy with others during the various stages of dementia. Case studies relative to resident; resident to spouse; and self gratification will be presented. The group will explore existing state guidelines as well as various facility guidelines and how they relate to achieving and preserving the rights of individuals living in a care community.

Bullying is not an activity that occurs only in children and adolescents. Once a person establishes a pattern of bullying as a coping mechanism, he/she continues to utilize that behavior throughout their lifespan. Bullying does occur in the congregate living setting of elder care, assisted living and other long-term care facilities. However, we, as caregivers, have failed to recognize it for what it is and address it appropriately. The first step to arresting this behavior in your community is to train yourself and your staff to recognize and re-direct it. Remember, bullying can affect your census and your bottom line.
