Definition – Intermediate Care

> “Umbrella” term> Describes an approach involving a

range of services working to common aims and principles.

> Supports recovery, mostly home based, at times of crisis.

Intermediate Care Demonstrators

> December 2008 – bids invited> National priorities of dementia, 7 day

access, staff training and development, pharmacy and reduced delayed discharges

The Art of the “Inbetween”

> Intentional design of something we could not easily describe

> All areas were working on this> Evidence increasing but more

required

The process

> Invitations to bid - Dec 2008> Short turnaround eliminated many> The enthusiasts prepared their bids> Partnership sign off essential> Selection procedure at JIT

The Sites selected – 5 + Orkney

> Ensured some overlap in topics to increase potential for learning

> All key priority areas were covered> Support from the JIT given at start

and throughout the project.

meet needs that can deliver change
