SHOWCASE: GREYS HOSPITAL PAEDIATRIC

DEPARTMENTS

The Department of Paediatrics at Grey’s Hospital works within the

Pietermaritzburg Metropolitan Hospitals Complex and includes the Paediatric

departments of Grey’s, Edendale and Northdale Hospitals. This allows more

efficient and effective utilization of the available resources and fits into the

ethos of the department. The departmental philosophy and vision reflects a

catchment population based focus with the principles of equitable access to

quality care within the resources available to the children within six districts

of KZN (Ugu, Sisonke, Umgungundlovu, UMzinyathi, uThukela and Amajuba).

Clinical governance activities and alignment with international, national and

provincial priorities aim to ensure that children across all ages get the best

possible health care, directed at their measured needs. 4

This process was started by Dr Neil McKerrow in the 1990’s. His visionary

leadership and credible successes has culminated in several influential

national and provincial accomplishments. In April 2013 he was promoted to

Natalia; where he heads KZN’s efforts to improve the health of children. He is

also influential in national policy and strategic leadership as the Chairperson

of the National Ministerial Committee on Morbidity and Mortality in Children

Under 5 (CoMMiC) since its inception in 2008. This has placed Paediatrics in

Pietermaritzburg at the “center of the universe”, and Grey’s Hospital

Paediatrics in a unique position to contribute significantly in improving care

for children in KZN and RSA.

The service delivery mandate of Grey’s Hospital as one of the tertiary level

hospitals in KZN has directed the development in Paediatrics at our hospital.

We have worked closely with Dr McKerrow towards success in this important

plan. The services we currently offer is substantially more than the past but

remain less than the needs of the children.

These services comprise: intensive care services for newborns and children

(NICU & PICU), tertiary in-patient and out-patient services by sub-specialists

and specialist paediatricians in general paediatrics, haematology, oncology,

infectious diseases, cardiology, neurology, child development, renal, liver,

respiratory, endocrine and genetic conditions in children.

These services are made possible by a mixture of dedicated, caring and

knowledgeable doctors and nurses of all ages, experience, expertise and

abilities. This group of people is in the opinion of many, the single most

important factor responsible for improvements in care as illustrated by

declining mortality (Figure 1) over time (2005 to 2013) at Grey’s and in the

Metro Hospitals.

Other factors that have contributed include improved understanding and

functioning in the health care system, leadership, innovation, clinical records

and record-keeping, improved data and data directed, evidence-based 5

decision-making, clinical protocols and care guidelines, ongoing training and

development of staff in several cadres, celebrating successes, cherishing

shared experiences and memories and finally trying to develop a culture of

caring, improvement and quality. We hope that our future dream of a “center

of excellence” in care for children and a legacy that will continue beyond our

time here is realized!

In future editions, I hope to show you the different facets to Paediatric care at

Grey’s Hospital with the wonderful people who make all of this possible by

going into each unit and telling you more about us. In this way I hope that you

will learn about our children’s’ needs and join us in campaigning for them and

contributing to our development of improved services.

HAEMOPHILIA

Sr Ramgobin and Sr Hemmero from the Out Patients Haemophilia clinic

accompanied by the Physiotherapist Jaenneitha Bramdaw created awareness

on Haemophilia Day Celebrated on the 16th April 2014. Mrs L Van Der Merwe

shared her challenges and personal experiences as a haemophiliac patient. A

special thank you goes to Chantal from Novo Nordisk and Adelia from Adcock

Ingram who sponsored the function. Topic discussed on the day were:What is

haemophilia, pain management , treatment compliance, psychosocial 12

care, building self-esteem, care of joints and muscles during a bleed, support

groups and the importance of reporting to the nearest treatment centre for

testing if rare bleeding disorders were observed.

CAREER OPEN DAY

On the 9th of May 2014 Grey’s Hospital held a career open day at the

Recreational Hall and the aim of the event was for school learners to be

exposed to tertiary Hospital services, bursaries and career opportunities

available in the health department and how these services benefit them.

These are the schools that attended the open day; Eastwood Secondary,

Sukuma High, Mehlokazulu High and Raisethorpe High.

CHILD PROTECTION WEEK 2014

From the 26th May to the 02nd June 2014 Grey’s Hospital had Child

protection week awareness, we visited Nogqaza Primary School. A message of

having the community protect their children was delivered with success.

MEET SOUTH AFRICA’S YOUNGEST DOCTOR

Sandile Kubheka is a 20 year old Doctor who began schooling at the age of 5

years and finished his primary schooling at the age of 10.He started high

school at the age of 11 and finished at 15 years old. Dr Kubheka started his

journey of being a doctor at the age of 16 when he began studying at the

University of Kwa-zulu Natal. His reason for choosing Grey’s Hospital was that

“Grey’s is a Tertiary Hospital where one can obtain and gain the best

professional Health Care experience possible”. He states “every generation

has a responsibility to do better than the previous one, and it is important to

know and understand your strength and weaknesses ,that way you can

accomplish more than what you are capable of.”

experience for the kids and it brought smiles to their faces.

TRIBUTE TO DR NIEL COLLINGE

1963 - 2014

Dr Neil Collinge started at Grey’s Hospital in 2007.

During his time with us he accomplished the following: He put into place a

number of policies and procedures acting in the best interest of the patients.

He helped change attitudes towards dialysis and treatment of renal patients,

and opened the Continuous Ambulatory Peritoneal Dialysis (CAPD) Clinic at

Grey’s hospital.

Dr Collinge saw the need for Renal services at Madadeni, and began an

outreach initiative which took him to Madadeni every second Thursday of the

month. He organized for the staff of Madadeni to come to Grey’s for basic

training in CAPD. To date 10 Madadeni staff members have been trained and

a CAPD Clinic opened, which allows Grey’s Hospital to do more.

Personally, he enjoyed life and food was one of his passions. He loved to

entertain family and friends and would go “all out” for a simple braai. He

loved to bake and would sometimes bring us a cake –“scrumptious to say the

least!”

He was a generous man – willingly giving of his time, knowledge and money.

He even bought paint for the CAPD Clinic, provided a computer and printer

and a BP Machine. His books were always available for both Doctors and

nurses alike to borrow and read and still are.

So, we bid a fond farewell to an exceptional human being, with a zest for life,

who championed the fight for better Renal Care at Grey’s Hospital.

He will be sorely missed.

ADOPT-A-DAY FEEDING PROGRAMME

The beginning of 2013 saw a group of concerned members of staff come together to address a very real and immediate need amongst our Planned Patient Transport (PPT) patients where they would be provided with at

least one meal as the journey undertaken by our PPT patients from the 19

far reaches of our province could take up to three days. They are more often than not, from poor and impoverished backgrounds and cannot afford the basic meal. They arrive at the clinics in a weakened and compromised state which is not conducive and often hampers any clinical intervention and achieving optimum medical benefit. The Institution is restricted by the fact that we are unable to provide meals for outpatients. The team was convinced that with a little creativity, innovation and determination, something could be done to alleviate this problem. With the blessing of our CEO, Dr. Bilenge, the team embarked on a campaign to enlist the help of the community. The response was phenomenal and within no time we had a full roster from Mondays to Thursdays. The Chase valley Ward Council, staff and family, local businesses as well as the All Saints Church happily agreed to come on board. We were looking for an average of 250 meals at the minimal cost of R350.00 per day. All Saints Church made a commitment to adopt every Wednesday of the month at Grey’s and to provide three sandwiches, fruit and juice to every outpatient, not only the PPT patients. Their commitment has grown significantly since then and they have varied their meals with soups, bread and fruit in the winter months. They also distributed four hundred blankets to our PPT patients last winter. In addition to making the sandwiches and meals, the members of the church distribute the meals on a Wednesday. A further 300 blankets were distributed by the All Saints Church on the 11

June 2014.

The Grey’s Hospital Adopt – A – Day program, now features on their annual

outreach business plan and the committee has a very fruitful partnership with

All Saints Church under the leadership of Reverend Ronald Hall. A motivation

was made to the ward council and was well received. The Chase valley Ward

council led by Mr. Dave Ryder adopted this program under their Health

portfolio for the ward. They view this as a deserving project to invest in and

he presented it to businesses and community members around Grey’s

hospital. The Ward council is now responsible for 50% of the project and has

allocated businesses and others to ensure that a meal is provided daily.

All remaining days are adopted by Grey’s Staff and family members,

demonstrating that we do indeed care enough to go beyond the call of duty

for our patients. The project has grown from strength to strength with the

invested interest of our community partnerships. Our patients have

responded with deep gratitude and the meal is graciously received daily. The

project is now self -sustaining and into its 2nd successful year. It is coordinated

at the beginning of the year by the team and a roster is drawn up and it runs

accordingly throughout the year.

The actual number of patients fed through this program is now between 300

to 350 per day. Grey’s Hospital has a vested interest in the holistic care of

patients and in customer satisfaction. We have been able to close a

significant gap in our service by forging community partnerships. The

committee is deeply grateful to the Hospital Management for encouraging

and supporting the innovative thinking. Grey’s Hospital is very grateful to all

our partners of the “Adopt - a - Day” project.

OUR NEW AND IMPROVED INTENSIVE CARE UNIT

THE OLD: The original ICU at Grey's Hospital was a six bedded open unit with just one isolation cubicle and had limited space. Amongst other things, the unit lacked proper isolation facilities as well as an emergency fire escape. It had just two dialysis ports and patients would be shifted around in order to access the ports. In order to accommodate an emergency escape, one bed had to be sacrificed to comply with Occupational Health and Safety requirements. The unit had no windows to allow for natural sunlight and patients were wheeled into the passage to derive the therapeutic benefits of sunlight. Despite the lack of space and the ever increasing need for refurbishment's and upgrades, the staff of ICU, under the exemplary leadership of OM Jenny Stewart, made the best of what they had at their disposal. Through their dedication and commitment they were able to render optimal patient care. THE NEW: The new Intensive Care Unit at Grey's Hospital was commissioned on the 18 March 2014. Although licensed for twenty one beds, there are currently only six beds operational. State of the art beds are anticipated to commission further beds with additional staff currently utilized in other units until the beds arrive. “The new unit boasts a huge, open patient area which is more than ample!” as the beaming Sr Stewart exclaims. Expansive windows that allow beautiful sunlight and a view of the beautiful outdoors have transformed the unit into a bright and airy place that is extremely conducive to the wellbeing and recovery rate of our patients. The patients’ satisfaction is evident in their positive feedback. This not only lifts the spirits of our patients but has also significantly improved the morale of the staff of ICU which has lifted the level of patient care as well. The unit has three isolation booths, two of which are fitted with their own ablution facilities, which is in keeping with the National Core Standards. Sixteen stations are fitted with their own dialysis port which means no more shuffling of patients.

New state of the art multiple-parameter monitors as well as ventilators were procured in the last eighteen months. The new ICU has its own seminar room which is used for the Advanced Cardiac Life Support and Basic Life Support courses and enables education and training to be conducted with staff on hand in the event of emergencies. The new unit is headed by OM Jenny Stewart who has been there since 1991. Sr. Stewart is an extremely "proud mom" as it were. She is however all too modest to accept any acknowledgment but attributes it all to her loyal and dedicated team. She is extremely humbled at the honour of having commissioned the brand new Intensive Care Unit at Grey's Hospital; it is not an honour that many will get to experience in a nursing career.

CRÈCHE EASTER EVENT

On the 17th April the kids were surprised with a goody bag full of treats. They

also enjoyed a story on why we celebrate Easter and an Easter egg hunt in the

kids play area. It was great seeing all the little smiles on the kids’ faces. The

kids enjoyed a good morning outside.
