Part 2 – Conduct a safety survey

Relevant to students completing either the workplace pathway or the simulated pathway.

Note: Read all instructions first before starting.

For this part of the practical assessment, you are required to apply consultation to facilitate participation of work team in managing area hazards. You will be doing this by conducting a Safety Survey in your workplace.

1. Conduct a Safety Survey to at least three (3) workplace personnel. Use the

Safety Survey Sheet Template provided in the Appendix section of this

workbook.

2. Use the Survey Results Table in this workbook to organise and interpret

the survey results.

3. Email a Report to your workplace supervisor/management detailing the

results of the survey. Your report must include:

a. The results of the survey; and

b. Your recommendations based on the survey results

When you send your report, consult with your workplace supervisor / management on which of your recommendations he/she would like to implement. Your workplace supervisor/management must approve at least one (1) recommendation in your report.

4. Send a Report to relevant workplace personnel. Your report must include the

following:

a. The results of the survey; and

b. Recommendation(s) approved by your workplace supervisor /
