How Provider Workload is Measured
Provider productivity is measured by RVU’s. These are determined primarily by 2 components:
◘ The E&M (Evaluation and Management) Code
◘ And Procedures (CPT and HCPCS codes)

Provider Documentation Considerations
838 doesn’t count anything outside of the S/O and AP modules in its automatic E&M calculations◘ Therefore, for example, vital signs entered by a technician in the VS
module aren’t counted even though CMS rules allow this
◘ This led to chronic under-coding
3.3 counts all CMS-allowed items, leading to more accurate automatic coding

Using a Utility AIM Form to Obtain Optimal (Correct) E&M Codes
More terms (e.g. symptoms, exam findings) is not always better
It’s where you place the terms that counts. Therefore,◘ Make the Electronic Record do the placing of terms for you
◘ Allow the provider to shape the note to allow easier reading of the patient’s condition, not to “maximize” the code

- ADM Errors are varied and have varied causes and solutions
- ADM Error Codes and fixes are located on the Field Services website- https://fieldservices2.saic.com/Report.aspx?Id=941
- Tier 3 is working on an updated version
- If desired, TIER 3 can requeue items that have not crossed.
- 120 error codes can be ignored. The 120 designation indicates a fixed ADM writeback error.

Rules for Pseudo-SSN Use
Newborns◘ Require pseudo-SSNs until a SSN has been assigned.
John Does◘ EWSR Phase 1 release notes Section 5.3:
The John Doe option allows you to register a patient whose identity is unknown. Once the patient's identity is determined, you can update the registration data using the Registration option.
For this type of registration, the system automatically inserts DOE,JOHN(#) as the patient name, 99 (All others, not elsewhere classified) as the FMP, and a pseudo SSN (8xxx-xx-xxxx). The JOHN DOE COUNTER (#) is assigned by the system based on a John Doe Count function that increments by one for each new John Doe entered (i.e., DOE,JOHN17; DOE,JOHN18) and is stored in the ADT PARAMETERS file."
