Financing Encounter
In the Emirate of Abu Dhabi, everyone has access to healthcare and freedom to choose their provider.
A system encompassing the full spectrum of health - protecting, promoting, sustaining and restoring services across the territory of the Emirate.
Providers are independent and predominantly private.
Quality driven by ambitious improvement targets set by the regulatory authority of the Emirate and reflected in the regularly monitored and published key performance indicators of the system.
An open system for all certified providers of health services delivers World-class quality care and outcomes in compliance with the highest international standards.
2
The health system finances itself through a mandatory health insurance for all AD residents.

Public health highlights

• Population One in five residents are Nationals of whom two thirds are under 30 and half under 198. Expatriates are overwhelmingly male and of Asian origin and predominantly aged between 20 and 408,9. A significant share are employed in construction and accommodated in labour camps. The introduction of mandatory health insurance in 2007 provided all residents in Abu Dhabi access to high quality care. Residence status is generally contingent on being employed, so there are very few retirement age or unemployed expatriates. The population has been growing rapidly in previous years, with likely temporary decline in 2009.
• Birth Fertility rates – the main driver of growth for Nationals –have declined for over 30 years13,14. The UAE’s Total Fertility Rate has declined from 4.4 to 2.3 per woman between 1990 and 2007. Declining birth rates are attributed to urbanisation, delayed marriage, changing attitudes about family size, and increased education and work opportunities for women.
• Death Mortality rates have also declined steadily13 over the past yaers. Infant mortality is now comparable with other developed countries6 and the WHO recently reported a decrease in the under 5 mortality rate from 15 to 8 per 1’000 live births between 1990 and 2007 across the UAE. In 2009, the diseases of Circulatory System caused the highest number of deaths, accounting for 24% of all death cases registered in the Abu Dhabi Emirate. External Causes of morbidity and mortality and Neoplasms are the second and third highest causes of death15,16.
• Injuries Abu Dhabi has one of the highest rates of injuries resulting from Road traffic accidents. They account for 14% of all deaths and are the leading cause of death amongst young males16,17. Speeding fines, free provision of child seats, and traffic
safety education programs are some of the actions being taken by government agencies. Occupational injuries are covered directly by employers, not by health insurance.
• Communicable diseases Rates of childhood communicable diseases are very low23, due to immunization programs targeting children aged <5 years17. Expatriates are screened for communicable diseases before acquiring residence status17.
• Non-Communicable diseases The Emirate has high rates of chronic diseases related to life style such as obesity, diabetes, and cardiovascular diseases. Cardiovascular diseases already accounted for a quarter of deaths in 2009. Adult Nationals were screened for cardiovascular risk factors in 2008 as a condition for enrollment in Thiqa insurance. Early analysis of results of this screening show obesity rates of 33% for males and 38% for females and high proportions of people at risk of diabetes and hypertension among UAE nationals over 1519. Without major changes, these rates are set to increase further as the young population ages. Individuals thought to be at high risk of cardiovascular disease are being followed up.
• Cancer caused 14% of all deaths in the Emirate in 2008. Breast cancer is the dominant cancer in Abu Dhabi18. Late detection of breast cancer leads to significant increases in mortality. Female adult Nationals aged 40-69 are being screened for breast cancer as part of their Thiqa insurance renewal. Education and awareness campaigns have increased screening rates for all nationalities
• Respiratory infections are the most common non-life threatening condition in the Emirate, accounting for over 18% of all encounters across all healthcare facilities28. Respiratory infections impact workforce productivity and quality of life.
• Population The population is concentrated on or nearby Abu Dhabi island9,10. Areas of growth in the short to medium term are expected to be just off the island (Khalifa City A, Mohammed Bin Zayed City, the islands adjacent to Abu Dhabi island) and the oldest city Al Ain. At the end of 2009 there were 0.4m National Thiqa members, 0.9m government-subsidised Basic members and 0.9m Enhanced members50. Daman administers Thiqa, it has exclusivity for the Basic product and one third of the competitive Enhanced market. Daman has recently given up its direct billlingexclusivity with the public healthcare provider SEHA.
• Demand for health services in the Emirate stabilised in 2009 – with the exception of ER encounters – following significant increases in 2007 due to the introduction of mandatory health insurance for Expatriates and Thiqa for Nationals in 2008 27. Significant growth in demand for services relating to chronic diseases, e.g. diabetes, is expected, with larger volume increases in outpatient settings. It is, however, estimated that there will also be aggressive growth in inpatient services relating to both diabetes and cancer36.
• Supply Despite growth in the number of doctors available to residents, it is estimated that up to 5,000 additional doctors and 6,500 nurses will be required by 2019. If churn rates remain at their 2009 level, this will require some 1,600 doctors and almost 1,800 nurses to be recruited annually35. There are productivity reserves within existing facilities, with average physician productivity in most hospitals lower than the US average37.Demand for inpatient services may require up to 2’000 additional beds within ten years beyond the current 3,642 beds42. 54 hospitals with temporary licenses suggests there is likely to be significant future development and investment in this area. Regulatory price adjustment encouraged movement in the home care services market – Sama Abu Dhabi Home Health Care Centre was licensed in June 2009.
• Capacity gaps Although waiting times have eased somewhat overall gaps remain, particularly in pediatrics, orthopedics, ophthalmology and gynaecology41. Overall bed occupancy rates vary significantly by facility, but have not increased in aggregate. Bed occupancy in ICU, NICU and PICU was consistently above optimal level of 75% throughout 200939.
• Reimbursement HAAD sets prices for the Basic product uniformly, adapted from the US-Medicare structure. Providers negotiate prices with Payers for Enhanced plans, generally as a multiple of Basic product rates. Thiqa is reimbursed at the level of Daman’s most generous Enhanced plan. Price levels are weighted towards outpatient care. In line with this, demand growth has been in ER and outpatient services whilst inpatient service levels have remained relatively stable from 2007-09 accounting for <2% of all encounters22. DRGs, anticipated in 2010, will likely increase inpatient reimbursement.
• Provider market Government-subsidised SEHA facilities provided 89% of all critical care bed days, treated 58% of all inpatients (4% loss from 2008) and 36% of all outpatients (8% gain)27,38. The largest independent groups are Al Noor with 12% and NMC with 8% of the outpatient market, respectively. Off the island, health services are concentrated in larger facilities36,40. International providers have come to Abu Dhabi, generally on the basis of a management service agreement, such as the Cleveland Clinic for SKMC and Johns Hopkins for Tawam.
• Payer market Mandatory eClaims for virtually all from January 2010 (www.haad.ae/DataDictionary) created a market for eClaims software. Dimensions Healthcare and Greenrain have invested to offer free solutions. This helps mature a competitive payer market, which has seen the first full year of operation of dedicated health insurer, Green Crescent, and entry of 3 insurers, 3 TPAs and 6 brokers.

Key indicators
Notes Population assumptions were adjusted from the Department of Planning, based on health insurance data; Physicians ,nurses and beds statistics in WHO and other reports are reported for different years ; Bed ratio calculations were adjusted, as the population in Abu Dhabi is young and is not expected to need to go to hospital as frequently as other, older population. To enable a fair comparison the bed ratio was adjusted by mapping the population age structure of Abu Dhabi to that of Germany, using German resource consumption profiles
