Clinical/Practicum Compliance Requirements

Physician Assistant Program

Incoming Students(First Year Students)

Please use this form as a guide, as all aspects must be completed prior to attendingclinical/practicum.

Physical Eam and Immuni!ation Forms

 You will need to have documentation of a physical exam completed within the last 12 months.

Students are reuired to complete the o!cial "#$P %mmuni&ation 'ecord and Physical (xamination

)orm.

 *his process may ta+e one to two months to fully complete so please begin the process promptly.

***If you have not started the Hepatitis B series this will take up to seven months to

complete, so it is highly recommended that you begin as soon as possible.***

•  *woStep PP- with negative results within the last year0 in addition, the PP- will need to be

current for the time you are in clinical.

•  *dap vaccine within the last 1 years

• )lu vaccine for the current u season

• $epatitis 3 series three vaccines and  a positive titer showing immunity

• 4aricella either the twovaccine series or positive blood titers showing immunity

• 55' either two vaccine series or positive titers showing immunity for each of the following65easles, 5umps and 'ubella

Seasonal %nuen&a )orm -ocumentation must be submitted by 7ctober 1st annually. *his is reuired for all

)all, 8inter and Spring clinical/practicum rotations.

"P' "erti:cation 5ust be the American Heart Association – BLS for the Healthcare Provider  'R the American

Red Cross – CPR/AED for the Professional Rescuer . 5ust be current. *he renewal date will be set according to the

expiration date of your certi:cation.

Screenings

results must e on &le one month prior to the start o" the clinical assignment+

%n order to become compliant prior to your clinical/practicum, you must initiate the screening process

four months prior to the clinical/practicum start date. *his will give you ample time to receive your
