FY2014 – 2015 saw Temasek Cares funding 15 programmes through 26 community partners that directly impacted 4,900 Singapore citizens. Since 2009, we funded a total of 82 programmes which directly benefitted 22,500 Singaporeans. In addition, as part of our continuing effort to build up community resilience, we distributed ‘Stay Prepared’ Starter Kits comprising N95 masks to 1.2 million households. We have committed a total of $31.6 million in programme grants to-date through the endowments from Temasek Holdings, dedicated to benefit people in Singapore. People are at the heart of these programmes.

People at Heart: Transformative Multi-Layered ProgrammesMdm Theresa d/o Anthony, 54, is one of the people who benefited from our grant to Temasek Cares – Care Close to Home, an ageing-in-place initiative. She is homebound, sharing a rental flat with her daughter, who had to work full time to financially support themselves. Mdm Theresa had a stroke and has multiple chronic illnesses like lupus and arthritis. She lacked adequate caregiving support and assistance to help her manage her illnesses including taking medications and attending medical appointments.

With the help of the Temasek Cares – Care Close to Home programme administered by the Agency for

Integrated Care, AWWA and Kreta Ayer Seniors Activity Centres, there is now a programme case manager who helps coordinate Mdm Theresa’s care needs and a health care aide to assist her with routine daily tasks like meal preparation, bathing and medication. The programme has helped Mdm Theresa to age-in-place.

The Temasek Cares – Project Dignity programme is a home-based palliative programme for people with advanced dementia. 88 years old Mr Jem1, who has advanced dementia, is benefitting from this programme. He is bedbound, has end-stage kidney failure and was a repeat hospital patient before the programme. The Temasek Cares – Project Dignity initiative, administered by Dover Park Hospice and Tan Tock Seng Hospital, has enabled him to be treated at home and also improved his well-being. This is the first holistic home-based palliative programme that addresses the physical, emotional and psychosocial needs of elderly persons with advanced dementia.

Mdm Marie Tan2, 33 years old, and her son, had no home to return to and no family support after her release from prison. She was emotionally unstable and battling a drinking problem. The Temasek Cares – Individual Transitional Aftercare programme provided her and others like Mdm Tan, a safe shelter, counselling, addiction management and employability support

services. This programme, funded by Temasek Cares, is administered by Yellow Ribbon Fund and iC@RE Hub. In Mdm Tan’s case, she was helped to find a permanent home for herself and her son. Mdm Tan’s case is typical of other former female offenders who have benefited from this first shelter and aftercare programme for women.

Infants are a special group of people. The Temasek Cares Kids Integrated Development Service (KIDS) 0–3 programme focuses on the needs of children from

vulnerable mothers and disadvantaged families. It brings together a multi-disciplinary team comprising doctors, nurses, therapists, social workers and other professionals to help these mothers and their children from birth to 3 years old. They provide intervention and support in the areas of health, care and cognitive development for both mother and child respectively.

Ms Jane Koh3 is a 19-year-old single mother with special needs. She did not have family support for her pregnancy and had to learn to cope, both emotionally and physically, to being a mother. Under the guidance of the Temasek Cares KIDS 0–3 team, Jane is now confident in providing the best care for her child. The programme also maximises the child’s developmental potential.

The Temasek Cares KIDS 0–3 programme is unique. Its team of professionals have been invited to distinguished international conferences in Hiroshima4 and Birmingham5 to share their experiences and this new model of care for mother and child.

These are some of the people at the heart of our transformative, multi-layered programmes.

People at Heart: Building Resilience of Singaporeans and the CommunityAgain with people at heart, Temasek Cares also funded the following initiatives to build up resilience in our community.

When Singapore experienced severe haze in 2013, many of us were not prepared to deal with the

situation. We recognised the need for the community to be better prepared for emergencies. Supported by Temasek Holdings and through the Temasek Emergency Preparedness (T-PREP) Fund, we partnered Singapore Power and Singapore Post to distribute ‘Stay Prepared’ Starter Kits containing N95 masks and useful emergency contact information to all 1.2 million households in Singapore.

We learnt that the Sewol ferry disaster in South Korea in April 2014 tremendously traumatised the school-going children there. This led us to want to be able to provide community-based trauma therapy for our children. More than 60 therapists, social workers and counsellors were trained in Trauma Focused – Cognitive Behavioural Therapy through the Temasek Cares Kids in Tough Situations programme administered by KK Women’s and Children’s Hospital. This programme was timely as we were able to provide trauma therapy to our students who were affected in the recent earthquake at Mount Kinabalu, Malaysia.

Research studies and consultations with Singapore medical specialists showed that chest compression-only resuscitation can significantly improve, as time is of essence, the survival rate of victims of cardiac arrest. This simplified form of chest compression-only resuscitation does not need mouth-to-mouth contact; is do-able and more acceptable by the public. We are working closely with the Unit for Pre-Emergency Care, and supported by the Singapore Heart Foundation, St John Singapore and Singapore Red Cross Society, to train more individuals in the community with this skill over the next three years.

Our community resilience programmes will be expanded in 2016, for example, establishing a trauma network for the wider community, and a central registry of community first responders.

People at Heart: Engaging SeniorsSingapore has an increasing ageing population and we pay special care to the seniors among us. In December 2014, we helped to facilitate a roundtable discussion on the topic of “Enhancing Social Inclusion and Civic Engagement among Seniors in Singapore”. Participants included the Council for Third Age, Employment and Employability Institute, Human Capital Singapore, Agency for Integrated Care, Singapore Nursing Board, Ang Mo Kio Constituency, and North East Community Development Council.

The focus was on community-based programmes where able seniors could care for the frail elderly. These would include providing regular health monitoring, escort for medical appointments, befriending and social groups. There was also a discussion on a time-based system to encourage the community to ‘pay it forward’. Work is in progress with various organisations to roll out such community or constituency-based programmes soon.

Special TributeI would like to pay a tribute to Singapore’s Founding Father, Mr Lee Kuan Yew. People were at the heart of all his policies and initiatives. Mr Lee transformed Singapore from a Third World to a dynamic First World country. He devoted his life to the welfare of Singapore and Singaporeans – always striving to improve their lives and to build a caring society. Temasek Cares continues

to be inspired by his vision and passion for needy and underprivileged Singaporeans.

ConclusionI would also like to express my appreciation to Temasek Holdings and Temasek Trust for their commitment and belief in the work that we do. in outdoor work and contribute to our community even during times of crises.

The aim of the distribution exercise was to provide an initial supply of masks to help Singapore families and residents to cope with emergencies or environmental risks such as severe haze and acute flu pandemics. In addition, they were also encouraged to start thinking about, and preparing for, various emergencies and unforeseeable events.

The ‘Stay Prepared’ initiative was supported by the Temasek Emergency Preparedness (T-PREP) Fund which was established by Temasek Holdings with an endowment of S$40m in March 2014. The T-PREP Fund was set up to help build the capability and capacity of the Singapore community to cope with emergencies.

Singapore Post and Singapore Power staff helped to distribute the ‘Stay Prepared’ Starter Kits to all households.

Temasek Cares provided AIR+ Smart Masks and AIR+ Micro Ventilators to more than 13,000 vulnerable children as part of the Temasek Cares ‘Stay Prepared’ initiative to help build the Singapore community’s resilience to cope with emergencies.

These children aged 7 to 14, are from low-income families or residing in Children’s Homes. The masks and ventilators were meant to provide these children with an initial supply of suitable masks for use during emergencies such as severe haze or viral epidemics.

13,000 vulnerable children received the first certified N95 mask that is suitable for children, to help prepare them for emergencies.

The AIR+ Smart Mask is the first certified N95 mask that comes in sizes that fit the whole family with the smallest size designed to fit children aged 7 years old and above. The AIR+ Micro Ventilator is an added feature and can be attached to the Smart Mask to help the wearer to breathe more easily by keeping the mask dry, cool and fresh.

This programme complemented Temasek Cares’ initiative in 2014 to distribute N95 masks to all households in Singapore.

Temasek Cares is partnering the Singapore Heart Foundation to raise the bystander response rate in cases of cardiac arrest outside the hospital. This programme aims to equip the community with basic resuscitation skills i.e continuous chest compression-only resuscitation, where studies have shown that it is effective as a life-saving skill. Temasek Cares will also work with the Unit for Pre-hospital Emergency Care to enhance the pedagogy on continuous chest compression-only resuscitation and the use of automated external defibrillators (AED) in the four official languages in Singapore so that more people can access and learn these skills.

Complementary to this, Temasek Cares will, in partnership with Singapore Red Cross Society, build up a central registry of people in the community who can be the first responders in an incident. The registry aims to include first responders trained in basic resuscitation, first aid, trauma therapy and other skills required during emergencies. The centralised information can better support the community during emergencies.

With rising life expectancies in Singapore, the number of frail seniors with multiple chronic conditions and complex care needs will increase significantly. To be able to age-in-place, seniors will require access to a range of aged care services in the community and at home.

Temasek Cares – Care Close to Home is a pilot programme that supports vulnerable seniors to age-in-place within the community. In this programme, Care Teams consisting of Social Workers and Health

Care Aides from Seniors Activity Centres in Ang Mo Kio and Kreta Ayer provide care coordination and services to vulnerable seniors living in 1 to 2-room rental flats in these two areas.

This community-based programme provides personal daily care services for the elderly, including physical and cognitive care (e.g. medical and nursing care), social care (e.g. befriending and financial support) and personal care (e.g. showering and toileting). The Care Team will also assist to coordinate the multiple medical appointments on the elderly’s behalf and escort them to these appointments.

The Temasek Cares – Care Close to Home programme is expected to benefit 1,020 seniors over 3 years.

Mdm Theresa d/o Anthony, 54 years old, suffers from multiple chronic illnesses, such as heart disease, lupus and arthritis. She also suffered a stroke a few years ago. As a result of her health conditions, Mdm Theresa needs frequent follow-up appointments at the hospitals.

Her youngest daughter, Hema, is her main caregiver, however she has to work to support the family as well.

Mdm Theresa was referred to the Temasek Cares – Care Close to Home programme for assistance. Under the programme, Health Care Aide, Nabilah Anuar, helps Mdm Theresa with personal daily tasks and also escorts her to the hospital for medical reviews. Nabilah also helps to ensure that Mdm Theresa takes her meals regularly, sometimes preparing simple dishes for her at home.

With all the support in place, Mdm Theresa is able to better manage her health conditions and now have a more optimistic outlook to life.

Persons with advanced end-stage dementia require specialised and extensive medical support as they are frequently unable to articulate their needs and discomfort. Families and caregivers are often at a loss on how to care for their loved ones.

Palliative care for persons with advanced dementia is an intervention that can help to alleviate these discomforts at their final stage of life.

In this pilot programme, there is a new specialised Advanced Dementia Palliative Home Care Team comprising a resident physician, consulting geriatrician/palliative physician, nurse clinician and a medical social worker. This team supports persons with dementia and their caregivers with scheduled home care visits. The caregivers are further supported through phone calls to help them in their psychological well-being, identifying symptoms and giving pain relief to their loved ones.

the-clock, 24-hour access to the Care Team so that in the event of emergency, assistance is rendered promptly.

The 3-year programme, a partnership among Temasek Cares, Tan Tock Seng Hospital, Dover Park Hospice and the Agency for Integrated Care, expects to benefit 550 persons with dementia and their caregivers.

Mr Jem*, 88 years old, is one of the first persons referred to the Temasek Cares – Project Dignity Programme when it started in October 2014. Mr Jem had a fall previously which was attributed to cognitive impairment caused by Alzheimer’s dementia. Since then, there has been a continued functional and cognitive decline, resulting in Mr Jem becoming bedbound.

A repeat hospital patient, Mr Jem also has end-stage kidney failure which added to his frail health. Given Mr Jem’s condition, a home care plan was discussed with his sons, who were sure that he would be more comfortable at home.

In the first home visit by the Temasek Cares – Project Dignity team, they found that Mr Jem had a widespread itch due to kidney failure that led to his irritability and poor sleep. His family were taught simple strategies for skincare. Once the itch was

managed, Mr Jem settled comfortably at home, with improvements in his well-being.

The Home Care team ensures not only potential problems are addressed early, but that Mr Jem is well-looked after and his caregivers, are also given the necessary support as well. Since being on the Temasek Cares – Project Dignity programme, Mr Jem has not required further admission to the hospital.

This family and marriage counselling programme is specially catered to the needs of Malay/Muslim community taking into consideration the language, cultural and religious influences.

The programme includes:• Comprehensive marriage counselling

services in follow-through cases referred from Syariah Court.

• Early intervention services for at-risk families to improve their relationships,

with the objective of encouraging the families to stay together, building up specialised resources in the area specific to the community.

• Training of counsellors in community organisations on marriage counselling and family therapy skills for Malay/Muslim families.

Under the 3-year pilot programme, 450 Malay/Muslim families are expected to benefit from the counselling services and 60 service practitioners will be trained in marital counselling and family therapy.

Temasek Cares works with partners – iC@RE Hub and the Yellow Ribbon Fund, to develop the Temasek Cares – Individual Transitional Aftercare Programme (iTAP).

This pilot programme provides a structural residential aftercare services in a secular setting for newly released female ex- offenders and those recovering from addictions (substance and/or behaviour).

Based on a ‘housing first’ philosophy, the transitional accommodation facility delivers a safe and affordable shelter for a minimum period of 6 months from the date of admission. The temporary accommodation is complemented with individualised support in areas such as emotional, psychological, employment, family reconciliation, financial aid, and addiction management.

With help and support during their journey of recovery and transition to normalcy after their release, this programme will empower them towards obtaining permanent housing,

Mdm Marie Tan*, 33 years old, mother of one, was referred to Temasek Cares – iTAP upon her discharge from Changi Women Prison. She had no family support and and had no place to stay upon her discharge.

When Mdm Tan was first enrolled in the programme, she was emotionally unstable and battling an alcohol addiction problem.

Under the Temasek Cares – iTAP, Mdm Tan was provided a place to stay, counselled, taught coping skills to face life’s challenges and connected to the Family Resource Centre Social Service Office for financial assistance. She also sought professional help to quit alcohol.

Mdm Tan is now prepared and ready to find part-time work and a permanent housing solution for her toddler son and herself. *Name has been changed to protect beneficiary’s identity

Temasek Cares – Individual Transitional Aftercare Programme

The Yellow Ribbon Fund – Temasek Cares Star Award provides financial assistance to needy ex-offenders by helping them to upgrade themselves and achieve self-sufficiency through further studies and eventually, gainful employment.

Recipients of the Award would also be guided and helped in their preparation to work with potential employers. The Balaji Sadasivan – Healthcare Building Capability Project offers study awards to help the Intermediate and Long-Term Care (ILTC) sector recruit and train more skilled healthcare professionals. Upon graduation, the award recipients will serve in an approved ILTC institution of their choice for an agreed period of time.

Since 2011, the project has given out 31 awards to train therapists, nurses and healthcare administrators. This project is administered by the Agency of Integrated Care and funded by the Balaji Sadasivan Endowment managed by Temasek Cares.

Elvis Hoe, a student with Assumption Pathway School, is a recipient of the education bursary from Temasek Cares.

The education bursary programme provides financial support to students, including those from low-income families, to encourage these students to attend school on a regular basis. In FY 2014–2015, 84 students benefitted from the bursary which helped them meet school-related expenses like cost of textbooks, uniforms and transport so that they can attend school more regularly.

This programme supports students, especially those with special needs in improving their numeracy skills through an enhanced syllabus and pedagogy from the school, assisted by an educational psychologist and a support officer.

Students with learning needs will be screened and assessed by the educational psychologist to ensure that the enhanced curriculum is accessible to them. At the same time, the students will also be helped by the support officer through small group intervention and one-on-one coaching.

This programme is expected to support 264 students with learning needs over the next two years.

The Temasek Cares KIDS 0–3 is a community-based programme, to minimise the developmental gap of children from vulnerable families aged 0 to 3 years old. Studies have shown that a neglectful or abusive environment could lead to an increased risk for impairment in learning, behaviour and health in the child’s later years.

mother and child. To maximise the child’s potential, the mother is coached on nutrition care and ways to stimulate the child’s cognitive development. This programme also provides the mother with pre-natal and post-natal care, as well as support for her social and mental well-being.

The first year of the programme saw the establishment of the design modality, praxis, engagement and implementation process, desired outcomes and research framework. The Community Health Visitor, a new job created for this programme, plays a central role in supporting the mother and the family through regular home visits. 50 mothers and 31 children were served during the first year.

The team also looked into establishing the Temasek Cares 0-3 KIDS Centre, an integral component of the programme, in Ang Mo Kio. This centre will complement the home-based services with resources,

antenatal classes, baby care and parenting skills, and other education necessary for the general well-being of mother and child.

The programme is expected to reach out to 600 mothers and children from diverse backgrounds in the vicinity of Ang Mo Kio over a three-year period.

Rabiah, a single mother to 4 young children, is a beneficiary of the Temasek Cares KIDS 0–3 programme.

The Temasek Cares – Education Bursary provides financial support to Eurasian students from lower-income families that do not qualify for existing financial assistance schemes offered by the Eurasian Association or the government.

In FY 2014–2015, 26 students from Primary to Junior College levels were awarded the bursaries. The bursaries helped the students to pay for their educational costs including school fees, purchase of uniforms as well as textbooks. Some of the beneficiaries shared that receiving the bursaries motivated them to study even harder to improve their grades, to make their parents proud, and to have the satisfaction of achieving their goals.

Azalea Tiara Oliverio, a student from Anchor Green Primary School, was motivated to study hard when she received the Temasek Cares – Education Bursary. The bursary helped Azalea to purchase educational items such as assessment books, school-related items as well as providing an allowance for her school transport and meals in school.

Dawn Yip has always been passionate about working with children so she is persevering to complete her studies in Early Childhood Studies at Temasek Polytechnic despite strong parental objections.

Dawn was particularly interested in helping children with developmental delays. She finds it fulfilling to bear witness to their progression and response to intervention programmes.

Dawn Yip’s interest in working alongside children with special needs was boosted when she was awarded the Ee Peng Liang – Special Needs Building Capability Project Study Award in 2014 to pursue the course.

In FY2014 – 2015, Temasek Cares committed $12,203,677 in grants to fund 15 programmes which helped improve the lives of disadvantaged Singaporeans and build community resilience to emergencies. The programmes, when fully implemented, are expected to benefit 4,900 direct beneficiaries. In addition, programmes under the T-PREP Fund1 touched the lives of more than 1.2 million households and vulnerable individuals in Singapore.

Our expense ratio2 for the year was 8%, which is significantly lower than the average for the last five years. This is due to a considerable programme grant required under the ‘Stay Prepared’ Starter Kits initiative to distribute the kits, each containing three N95 masks and emergency contact information, to all households in Singapore. Excluding this programme, the expense ratio would have been 12%, in line with previous years.

To support the growth in programme grants, management expanded the Temasek Cares team, increasing the headcount to provide more support in the areas of programme development, programme monitoring, and corporate communications. Management also invested more in technology to better manage key processes in endowment tracking, funds disbursement and programme monitoring.
