Bereavement Leave

Sick Leave Bank• Employees are automatically members of the Sick Leave Bank (SLB), unless they
opt out. – May opt out/in the first 10 days of each contract year– When SLB drops below 300 hours, all members will be assessed 1 day
• To use Sick Leave Bank:– All of your own leave must be exhausted– 5 days must be contributed to current condition– Fill out the forms
• submit by the 15th of each month (apply additional months for extended conditions)
– (except Dec- forms due by the first Friday in December)• Submit to GEA (1116 13th Street- fax 970-353-4199)
– Doctor’s note should indicate a “RELEASE to Work” date– The SLB Committee will determine eligibility.– Maximum of 70 days available each year– ONLY for employee’s condition- not family
