Easy-to-Use Home Telehealth Monitor A tablet-based telehealth monitor, provided by VA at no cost, connects you at home with your local VA Medical Center or Clinic.

InformationfromhealthdevicessuchasbloodpressureandbloodglucosemonitorsiselectronicallytransmittedtoyourCareCoordinator.ThisinformationisthenreviewedbyyourphysicianandVAcareteam,whomayrecommendchangesinyourtreatmentorlifestylesuchasdiet.Theycanalsoadviseyouoftips,appointmentsforvisitsandmore.

VAHomeTelehealthsecurelystoresyourinformation,soitcanbeaccessedatanytimebyyourCareCoordinatorandcareteam.Thisenablesthemtoprovideyouwiththerighttreatmentintherightplaceattherighttime—andforyoutocontinuetoliveinthecomfortofyourownhomeandcommunity.
