A PROPOSITO DE …Caso clínico:
Mujer de 85 años que acude a consulta con clínica de malestar general, dolor torácico de características pleuríticas, disnea, tos sin expectoración y febrícula de hasta 37,5º
AP: Cardiopatía isquémica, hipercolesterolemia, HTA, litiasis biliar, osteoporosis postmenopáusica
EF: hipoventilación global, crepitantes inspiratorios diseminados
Rx: derrame pleural bilateral, con infiltrado basal izquierdo.
JC: NAC atípica
¿Cómo debería ser el seguimiento ambulatorio de esta paciente?

¿Y en los niños?
No se requiere en los niños previamente sanos y que se están recuperando bien Considerar en:- neumonía redonda- Colapso- Persistencia de síntomas
- No es necesario en los niños que se recuperan sin problemas tras un episodio de NAC
- Realizar tras 4-6 sem del dxco en:- pacientes con recurrencia
mismo lóbulo- pacientes con colapso lobular
en la radiografía de tórax inicial con sospecha de anomalía anatómica, masa, o aspiración de cuerpo extraño
No es necesario en niños asintomáticos con NAC no complicada. Considerar 2-3 sem después ttº en:- Neumonía recurrente- Síntomas persistentes- Atelectasia grave- Infiltrados inusualmente
ubicados- Neumonía redonda
Conclusiones
¿Cuándo?Entre 4 – 6 semanas
¿A quien?- > 50 años- Varón- Hábito tabáquico
No realizar si buena evolución
Considerar en:- Persistencia
síntomas- Recurrencia- Neumonía redonda- Sospecha de otro
proceso
