Rock Eagle 4-H Camp ---- March 5-7, 2018

Monday, March 5th

7:00am Students must be at school

7:30am Buses leave to Rock Eagle

Wednesday, March 7th

12:30pm Depart from Rock Eagle

2:00pm Arrive at AMMS

cover any costs incurred for damages caused by

a student to cabins, equipment, or any other

SHS 1 Form - Authorization for Student to Carry needed for:

1. Aceraninophen (Tylenol)

2. Antiacids (Tums)

3. Aspirin

4. Cough and Throat Lozenges

5. Ibuprophen (Motrin)

6. Oral Antihistamines (Benadryl, Claritin, Zyrtec, etc)

7. Epi Pen/ Audi-Q

8. Rescue Inhaler (Albuterol)

9. Insulin

ALL MEDS must be in baggie with SHS-1 form whether over the counter

or prescription. If prescription, then the SHS-1 paper must be signed by

physician.

If the MED is needed until the last day before the trip, then make sure

you have the baggie and the SHS-1 signed here at school by 2/16.

NO energy drinks

Communication

Groups are in classes outside and will not be

near a telephone during most of their day.

Cell phones do not always get great reception.

If you need to reach your child you can call 706-

484-2834 during the day or leave a message

after business hours.

In the case of an extreme emergency after

business hours, you can reach the night guard

at 706-484-2821.

Vending Machines available, but

rarely used. Water fountains are

available throughout the camp.

Dress Code and What to bring?

We do adhere to the school dress code while on

the trip for the most part. Students are allowed

to wear hats throughout the trip.

We will be there in March! Please send layers.

There is a packing list in your packet, please

follow it! Students can bring bug spray as well.

Online Permission Form

Student’s ID number (students know this number – it is their lunch number)

Students must also sign behavior agreement

After completing the online form -

send your cash or check (payable to AMMS) with your student

in an envelope to the front office no later than February 2nd.
