Overview:

• The Supplementary Health Plan has a $2,000 deductible each benefit year.

• This means the first $2,000 of eligible health claims are paid by you.

• Once the $2,000 deductible has been met, all eligible health expenses will start to be

reimbursed through the Supplementary Health Plan.

• The first $2,000 of eligible health claims, which were paid by you (satisfying the deductible),

can be submitted to your HCSA for reimbursement.

• Your HCSA can also be used for expenses not covered under the Supplementary Health Plan.

• HCSA allocations will be made by Husky July 1st

Considerations

Additional Information:

• The policy number will remain as 51691 and your employee ID will be your ID # into retirement.

• The Costco Pharmacy program remains in place for both retiree benefit plans.

Considerations:

• At age 65, each province provides Seniors Coverage, through provincial health care.

• Review your previous two year claims history.

• Do you have coverage through another plan?

• Are you comfortable submitting your claims to both plans and monitoring your HCSA

balances?

• Consider the future health needs for you and your eligible dependents.
