In this season of acknowledging others, I would like to thank the INSPAN
Board of Directors for their support and guidance in coordinating the 2013 Fall
Update. I would also like to thank the board for their flexibility with scheduling
due to my family health emergency.
As the INSPAN President, I would like to recognize the numerous competent
perianesthesia nurses who have faithfully administered care from preadmission
testing, preoperative arena, and phase one or phase two; whether for a few
months or several decades. Each perianesthesia nurse is an asset in their
setting!
This fall the INSPAN Board provided a half day offering. The evaluations
were positive regarding the program. However, the results were mixed on half
day compared to a full day offering. We are considering various options for the
Fall Update 2014. Please feel free to email your suggestions for Fall Update
2014. My email will be provided at the end of the message.
My goals for this presidency are to:
1. Involve more nurses in INSPAN.
2. Provide educational opportunities for the broad spectrum of peri-
anesthesia nurses.
3. Promote networking opportunities to share ideas and concerns
among perianesthesia nurses.
4. Mentor/encourage perianesthesia nurses to excel even further in
their respective areas of interest such as research.
5. Encourage every perianesthesia nurse to please continue to focus
on your patients/your purpose with passion and continued perse-
verance.
Healthcare has faced challenges in recent months. Additional opportunities
will present in the days ahead. I know each nurse is acutely aware of this fact.
However, I am not addressing the fainthearted. Recently, our family faced sev-
eral medical situations. We experienced “being on the other side”. Sometimes
this journey brought familiar faces and locations, other times we were in
“uncharted waters.”
As a seasoned nurse of thirty three years I would just like to share my heart
with each reader. I would like to admonish each nurse reading this to personally
rekindle your original passion for nursing. What was your motivation for wanting

Charlotte West BSN, RN, CPAN One of the common question submitted to the ASPAN Clinical Practice Committee, “Is there a minimal length of stay required for Phase I even if patient meets criteria for discharge? Ex: pa-
tient meets all criteria for discharge to Phase 2 in 20 minutes.” The criteria set up by the institution in collaboration with anesthesia department. The ASPAN Perianesthesia Nursing Standards and Practice Recommendations 2012-2014 state “Discharge criteria should be developed in consultation with the anesthesia department using the above assessment parameters. Discharge criteria must be approved by the department or anesthesi-ology and the medical staff. The registered nurse will adhere to institutional policy for patient re are the standard you would follow. When a patient meets discharge criteria the patient may move to the next level of care.” This relates to both Phase 1 and Phase 2 care and discharge.
The American Society of Anesthesiologists (ASA) has updated some of the practice guidelines.
The routine requirement for urination before discharge should not be part of a discharge protocol and may only be necessary for selected patients.
The requirement of drinking clear fluids should not be part of a discharge protocol and may only be necessary for selected patients, determined on a case-by-case basis (e.g., diabetic patients).
As part of a recovery room discharge protocol, all patients should be required to have a responsible individual accompany them home.
Patients should be observed until they are no longer at increased risk for cardiorespiratory depression.
A mandatory minimum stay should not be required.
Discharge criteria should be designed to minimize the risk of central nervous system or car-diorespiratory depression after discharge

As healthcare changes it is important for nursing to use evidence-based research to qualify the care in the perianesthesia arena.

Perianesthesia Nurse Awareness Week (PANAW)—what a wonderful week it is! As we celebrate this week,
February 3-9, our theme is PeriAnesthesia Nurses: Caring Every Step of the Way. I hope you take some time to
reflect on all this embodies. Through the entire continuum surrounding surgical, diagnostic and interventional
procedures requiring anesthesia, sedation and analgesia, the perianesthesia nurse is ever present. Further, our
steadfast commitment to honesty, integrity, and excellence has helped keep nursing ranked as the most trusted
profession in the country!1

Have you looked at the latest ASPAN Standards Book? Standards are re-
viewed in the odd years and published in the even years. The interpretive
statements, added as sidebars, clarify wording. For example, describing
what is meant by “critical elements”. Equipment as needed now includes
capnography. Visitation clarifies that “family” is whoever the patient desig-
nates, not just a blood relative. OSA in the Adult Patient is now Practice
Recommendation 10. One of our component members, Kyra Hiatt, partici-
pated on a work team, and is recognized in the credits.
Online, at the ASPAN website, you can access Practice Guidelines, print
them off and post in your work area. There is a fatigue checklist, and did I
mention the JBI website for gathering evidence? Initially, we had issues
with spelling. JBI is based in Australia and uses British spelling. Now, there is
a conversion to help you find what you are looking for. Please contact
ASPAN, or me, if you need assistance. Remember, the first step to making a
change is to gather the evidence.
A Position Statement on Social Media and Perianesthesia Practice has been
approved. Today’s technology makes it possible to stay connected through
the media. How many of us have our phone within reach, while we are
working? (Eye rolling is allowed, but only once.) Please be aware that, if you
are on your device, you are not available to assist a coworker. Law suits
happen, and phone / computer records are pulled to look at individual ac-
tivity. Follow your institutional policies and be professional. Protect your
personal information and avoid commenting negatively on the happenings
in your work place.
I learned something else, with regards to our practice. Did you know that,
from a safety standpoint, privacy is secondary, to the safety of the patient
in the PACU? We pull the curtains, and we receive a second patient. ASPAN
says the patient should be visible at all times in Phase I care. We need to be
inside the curtain, or have the curtains opened so we can view the patient
at all times. This can be a challenge for Ambulatory Centers who fast track.
Wow! We have raised the bar on safety and confidentiality. We are moving
on with Unwanted Sedation.

otherwise are real and very present. However, you are a committed professional, capable of focusing on
the task at hand. We are privileged to be caring for patients and their families at a very vulnerable time.
