Southmead ICU Burns Fluid Resuscitation Guideline

Lund-Browder Chart(Discount Erythema)

Estimated TBSA:

Revised Baux Score:

(Age + TBSA Burn + 17* (*Inhalational injury: 1 = yes, 0 = no)

Burn Area Assessment:

Use the modified Parkland Formula (3ml/kg/%TBSA Burns) to calculate an estimate of the amount of fluid required in the form of Hartmann’s over the first 24 hrs from time of injury

If

Urine Output or Blood Pressure targets not achieved: Albumin Rescue

Add 100 ml of 20% Human Albumin Solution (HAS) over 1 hour to fluid regimen

NB If patient cardiovascularly unstable consider other co-existing injuries and the need for alternative blood products.

No maintenance fluids required

Use 100ml 20% Albumin as fluid rescue (if available)

Urine Output:0.5ml/Kg/hr

Monitoring:

Fluid resuscitation with Hartmann’s solution

Adults > 15% TBSA burns require IV fluid resuscitation

3mls x weight(kg) x %TBSA burn = Total Fluid Volume (TFV) over 1st 24hrs from time of injury

Fluid for 1st 8hrs TFV ÷ 2 = Fluid for 9-24hrs TFV ÷ 2 =
