Bid Opening
The following requirements may apply:
• Workforce plan: If the apparent low bid (ALB) is $5 million or more, ALB must submit a workforce plan;
• On-the-Job Training: Review Special Provisions of the proposal to determine if the project has an on-the-job training goal; and/or
• Tribal Employment: Review the Special Provisions Relating to Indian Employment in the proposal to determine if there are tribal employment requirements.

Prior to Construction and life of the project
• Contractor Employment Data Form (EEO-12): If federal-aid project, the prime must submit an EEO-12;
• Monthly Employment Compliance Report (EEO-13): If federal-aid project, the prime must submit EEO-13s throughout the life of the project;
• FHWA 1391 Report: must be completed by all contractors that work onsite the last pay period in July
• OJT Trainee Application: due prior to construction or within 30 days of employee start date;
• OJT Mentorship Agreement: each agreement must be reviewed and approved by OJT Program Coordinator;
• OJT Departure Form: due within 15 calendar days of an OJT trainee voluntarily or involuntarily leave the company; and/or
• Indian Employment Tracking Form: due within 30 days of project completion

• $1 per hour: approved OJT trainee
• $5 per hour: approved OJT trainee + graduate of MnDOT Sponsored Training Program
• $10 per hour: approved OJT trainee + graduate of MnDOT Sponsored Training Program + assigned a mentor
