Phase 3 Quarter 1 Remediation
Process & Timeline
• CNYCC has completed review of partner submissions for Q1 PAs & VBP activities
and will distribute Q1 initial scorecards & remediation webforms on Monday, 10/29
• Partner organizations will have until 5:00 PM on Monday, 11/5 to submit any
remediated documents• For Q1 only (like last year), CNYCC will accept net-new submissions during remediation
• Check scorecard for list of activities that require remediation as well as comment
that indicates the reason why remediation is required• Submit remediated deliverables via the mechanism identified in the partner reporting guide
(typically via webform except for surveys, AEP rosters, workforce reporting)
• Note: Please check Appendix 1 of your Phase 3 Project Addendum to
determine whether your organization was assigned VBP Infrastructure
Activities (due in Q1) or VBP Readiness Activities (not due in Q1)

What if your organization
has not yet received a
Phase 3 Project Addendum?
• Some CNYCC partner organizations that were designated as non-safety nets have
not yet received their Phase 3 Project Addenda
• Non-billing CBOs: upcoming communication re: safety-net hubs (early next week)
• Other partner organizations: partner-specific follow-up underway
• However, if you have not received your Phase 3 Project Addenda, you have been
sent a copy of Appendix 1 & 2 listing your VBP Activities & PAs
• If you submitted for any Q1 VBP activities or PAs, you will receive a scorecard &
remediation webform (check scorecard to see if any submissions require remediation)
• If you did NOT submit for any Q1 VBP activities or PAs (most common), you will NOT
receive a scorecard but you will receive a webform email (please disregard)
