Lesson learnt
WHAT IS A CONFINED SPACE?
A space that:
• Is large enough and so configured that an employee can enter bodily and perform work;
• Has limited or restricted means of entry or exit;
• Is not designed for continuous human occupancy.
Examples at Rig site…
• CELLAR AREA
• Tanks
• Pits
TANK
PIT
Main Hazards:
Toxic Atmosphere
Flammable substances
Oxygen enrichment / deficiency
Flowing Liquid or free flowing solids
Excessive heat
Hazards effects:
Fatality
Serious injury from fire or explosion
Loss of consciousness or asphyxiation from dangerous fumes etc.
Hazards
A safe system of work must be in place.
As per Saxon procedure for confined space (CAN-OFS-5.2-PRO-005:
Rev 02),
A competent person MUST have atmosphere tested the area. The test
must include the amount of oxygen, LEL % & H2S etc.
Oxygen must be between 19.5% and 21% for entry without a or SABA.
If the oxygen level is less than 19.5%, you must wear a SCBA or SABA.
If the oxygen level is more than 21%, the area must be evacuated
immediately and entry is not permitted.
Read and understand the Job Safety Analysis.
Obtain and use the correct PPE.
Inform your supervisor.
Each person must wear a Portable Air Monitor (PAM).
Do not enter alone ! FOR ANY REASON.
Watchman is mandatory.
What to do before entering a confined space ?
PAM
Precautions may be needed
• Tripod and fall arrestor
• Forced Air Ventilation
• Body harnesses
• Explosion proof lights
• Respiratory protection (SCBA or SABA)

Gas Test Results
Gas Test Results
Gas Test Results
Atmospheric air testing not carried out before entering confined space
Watch Man not available
Blower or equipment not available for proper air ventilation
Sometimes oxygen level is within limits but it is required to avoid
suffocation
Rotation of employee not carried out as per defined time period
Persons working inside confined space must switch their location
with watchman
Area not barricaded by caution tape or by hard barricading
Confined space entry checklist not used just Oxygen level written on
PTW
Permit end time not written on PTW form
Old permits not removed from PTW Board causing confusion
Previous Deficiencies related to Confined Space Entry
Evacuate the confined space
Don’t attempt the rescue yourself
Get help immediately!
Use emergency contact procedure
Give details of confined space location
Lead the professionals to the incident
Stay outside the confined space
What to do in case of any emergency?
Painting in Confined Space causes fire – 2 Fatalities
Employees in day shift painting in confined space continuously for
10 hours.
Lets watch a small video.
What went wrong?
Paint mixture was flammable.
Lighting equipment was not explosion proof
No ventilation and only one excess hole
Painters and labours enter confined space without any air space
monitoring.
No electrical connections within 50ft of hazardous atmospheres or
painting operations.
Employees not trained to identify confined space hazards.
