โรคติดต่อติดเช้ือ (Infectious Disease-[Mite-borne Disease]) น าการติดเช้ือโดยแมลงในกลุ่มไร (Mite_Genus Leptotrombidium)-{ตวัอ่อนไร-Chigger-Mite} เป็นพาหะน าเช้ือแบคทีเรียในกลุ่มริกเกต็เซีย (Rickettsia) ชนิด Rickettsia tsutsugamushi
หรือ Orientia tsutsugamushi โดยผา่นโฮสตใ์นสตัวก์ลุ่มฟันกดัแทะ (Rodentia)

About 1 million cases
occur each year
Over 1 billion people
at risk, world wide.
Scrun typhus also occurs in India, Sri Lanka, Pakistan, coastal Australia (Queensland) &
islands of Southwest Pacific, parts of middle east countries [Oriental-Australian region]
• During the World War II, Scrub typhus was the second most
important disease behind malaria

• In chigger-mite, O.tsutsugamushi is transmitted
transovarially (from adult female to egg)
[Transovarian Transmission-ToT] as well as
transtadially (from egg to larval stages & to adult stage)
[Trans-stadial Transmission-ToS]
Orientia tsutsugamushi

Pathogen: Orientia tsutsugamushi
Orientia tsutsugamushi is a gram-negative,
rod-shape bacterium (cocco-bacillus)
[an obligate intracellular parasite of humans]
It is transmitted to humans and rodents by
the bite of larval (chigger) mites

Vectors: Trombiculid-Leptotrombidium Mites
Chigger VS Adult Mite
(0.1 mm) (1 mm)
• Chigger-mite is a hexapod with 3 pairs of legs
• Adult mite has 4 pairs of legs.
• Trombiculid mites are soft bodied mites, varied in colors
(from white, yellow to red) & shapes (elongate, oval to round)
• Of 500 known Trombiculid species in Asia,
~ 300 species are found in Southeast Asian
Trombiculid Mites of Thailand
• Since then only 3 species added in 1986 & 1997
•Leptotrombidium harrisoni & Leptotrombidium Andrei Thailand
(Species described by Tanskul & Gingrich, 1986)
•Leptotrombidium chiangraiensis: Thailand
Newly described species by Tanskul & Linthicum, 1997)
The most comprehensive checklist of the Trombiculid mites of
Thailand was published by P. Tanskul, in 1973(Department of Entomology, Medical Research Laboratory, SEATO Medical Project, Bangkok, Thailand)
• It contained 27 genera & 121 species of chigger-mites
• It listed 31 Leptotrombidium spp. found in Thailand

• Rodentia is a group of small mammals characterized by 2 incisors
in the upper & lower jaws, [the sharp incisors are used to gnaw wood, break
into food, & bite predators] followed by a gap (disatema)

In SE Asian countries & Thailand,
the most common rodent species:
• Animal-hosts for “Trombiculid mites” includes mice, rats, squirrels, hamsters,
guinea pigs, chipmunks, gerbils, gophers, beavers, porcupines, prairie dogs, groundhogs, etc.

Scrub Area
Scrub Area • Mite Islands usually close-related with
the animal host (rodent)’s habitats,
• Adult mite’s habitats are known as “Mite Islands”
• Mite Islands are usually found in scrub areas
along the edge of forest, cultivated-rice field,
transition area between orchard and village,
grassland areas, margins of stream, etc.
The name of this disease was originated from
the areas
where “Mite & Rodent Habitats” were found
where human obtained the disease by
chigger-mite’s bites causing “Scrub Itching”

Clinical Signs & Symptoms: อาการของโรค In humans-scrub typhus patient, the most common clinical signs &
symptoms are reminiscent of other infectious diseases, (dengue fever,
typhoid fever, murine typhus leptospirosis)
High fever, headache, myalgia, (มีไขสู้งร่วมกบัปวดศีรษะและปวดเม่ือยตวั)
Clinical Treatment: การรกัษาโรค
Doxycycline
In General, scrub typhus patients respond well
to appropriate antibiotic therapy, especially to
doxycycline, chloramphenicol
In severe cases, symptoms could lead to enlargement of liver & spleen
(hepatomegaly, splenomegaly), lymphadenopathy, circulatory failure, pneumonia,
& finally resulting in death

are Skin Rash & Eschar

• Eschar usually appears in
• chest area,
• arm-pit,
• belt-belly,
• groin or private areas,
• any areas of soft skin
“Eschar” = cigarette burn scar

Personal Protection from Chigger Bites
Personal Protection Measures:•Wearing loose-fitting clothing,
•Avoid sitting or reclining on the ground,
•Tapping pant & shirt,
•Soaking & spraying cloth with repellent.
• Most efficient & practical method to control chigger mite is the use of
chemical compounds, either in the form of insecticides or repellent.

method [but it is usually insensitive & non-specific]

2. Soil Samples: Berlese Funnel Extracting arthropods from soil & litter samples.
[It work on the principle that insects respond negatively to light.
Light source is used to force the arthropods to move downward,
& fall into a funnel and then into a container of ethanol]
3. Animal-Host Trapping: It’s the “most effective & reliable method”
to ensure of collecting chigger-mites
which fed on their hosts

Would it be possible that the occurrence of scrub typhus cases,
(which usually be found in the rural-natural field-areas), is moved from to sub-urban
& urban proximity?
Of 39 scrub typhus cases in Meuang
District, 16 cases were occurred in the
peripheral zone/area of the city:
ต าบลหนองระเวยีง 6 ราย;
ต าบลหนองใขน่ ้า 5 ราย;
ต าบลสรุนารี 5 ราย While cases reported around proximity
of the City-area/zone were accounted
for 1/3 of total number of case-reported
or about 14 cases:

Research on Leptotrombidium Chigger-Mites Vector
Collection of Leptotrombidium Chigger-mites can ONLY be
done by removing them off trapped-animal (rodent)-hosts
• Taxonomy of Leptotrombidium chigger-mite (vector of
scrub typhus) is extremely difficult
[It can ONLY be done on the larval or chigger-mite stage]
Taxonomic Characters
Dorsal/Ventral Aspects of Leptotrombidium deliense (Walch)
Taxonomic characters of Leptotrombidium chigger-mites can only be
accomplished by measuring of key morphological parts & counting
numbers of setae & bristles with (@100x magnification
[objective lens] under the compound microscope) !!!
Maintaining the colonies of scrub typhus-infected Leptotrombidium chigger-mite
is even more difficult with a very tedious & laboring task, required experiencing
technical skill to rear immature-mites
These infected chigger-mite required the “Animal Passage Procedure”
[feeding on lab-mice in order to receive protein source for developing into the next stage]

be lower than the actual cases occurred, due to
Lack of experience to diagnose the disease
Nonspecific signs & symptoms
Lack of the unique characteristic of “eschar” as
appeared in scrub typhus patients
Availability & prescription of the antibiotic drugs
Therefore, the actual number of scrub typhus cases would be much higher

Scrub typhus vaccine candidates were first developed by the team of the US-Naval
Medical Research Center (NMRC), led by Drs. W.M. Ching & A. L. Richards,[Rickettsial Diseases Division, Viral & Rickettsial Diseases Department, NMRC-WRAIR, USA]
Initial evaluation of scrub typhus vaccine candidates using the
intra-peritoneal (IP) injection [Needle Challenge Procedure] in mice,
Found to provide adequate & satisfactory protection.
Later, those scrub typhus vaccine candidates was assessed &
evaluated for their protection efforts on ICR mice using
“Natural Challenge Procedure”
[using O. tsutsugamushi-(scrub typhus)-infected chigger-mites] {This procedure was conducted by Ectoparasite Laboratory Section, Entomology Department, Armed Forces
Research Institute of Medical Sciences (AFRIMS), United States Army Medical Component, Bangkok, Thailand}
Although the results showed that there were some protection,
But, it failed to reach the level of standard protection as required

Avoidance contact with the animal hosts (Rodent Nest)
especially for
Outdoor Activity: cutting-mowing-harvesting-gardening field; picnic
People who are at risk: Farmers; Field-workers; Villagers
Medical Personnel: Physicians & Nurses
Monitoring the dispersal of scrub typhus cases/patients
