2015-16 Eligibility Criteria

• Local presence and programming

• Incorporated for 3 years or

longer (prior to Jan. 2012)

• Board of directors with at least

3 members, less than half

receiving any kind of

compensation

2015-16 Eligibility Criteria

• Provide most recent IRS Form 990 (or proforma 990)

• Total reported revenue of $50,000 or greater

• Overhead percentage of 35% or lower

2015-16 Eligibility Criteria

• Have a policy and demonstrate a practice of

non-discrimination as it relates to the operation of the

organization

• Must participate with United Way NCA in the CFC of the

National Capital Area and, if eligible, in the

Commonwealth of Virginia Campaign (CVC).

• Log-in info is emailed the week of Oct 27

• Last year’s login will be active (use ‘forgot password’)

• Each person can create their own account

• Use ‘secret key’ to access an organization’s application

• More than 1 person can work on the same application

• 1 person can work on multiple applications

Review all information to ensure accuracy!

Required Documents: IRS Letter

Letter of Affirmation or Letter of Determination • Most recently received from the IRS with the current name

and address for the applicant organization

• Must have language granting your organization tax-exempt

status (26 U.S.C. 501(c)3) and include the EIN number

Chapters/Affiliates or Group Exemptions

• Requires additional documentation from parent and/or affiliate

• Instructions provide more information

Required Documents: Form 990

Most Recently Completed IRS Form 990 • Must be complete and the same as filed with the IRS

• Cover a fiscal period ending June 30, 2013 or later

• Be signed and dated by an officer of the organization

• Include all applicable statements and schedules (which are

checked off on pages 3 and 4)

If you filed a 990EZ or 990N (e-postcard):

• Must submit a complete Proforma 990 for same fiscal period

Proforma 990

Must cover a fiscal period ending June 30, 2013 or later

Includes the following sections:

• Page 1: Items A-M; Part I lines 1-4 only; Part II (signature block)

• Page 7-8: Part VII, Section A only

• Page 9: Part VII, Statement of Revenue

• Page 10: Part IX: Statement of Functional Expenses

• Page 12: Part XI: Financial Statements and Reporting only

Must also submit a copy of the 990EZ or 990N that you filed

with the IRS

Online Application: Revenue & Overhead Percentage

(Management & General Expenses + Fundraising Expenses) X 100 = Overhead % Total Revenue

Required Documents: Audited Financial Statements

If IRS Form 990 (or Proforma) shows revenue of $100,000 or

more:

• Must submit audited financial statements covering the same

fiscal period as the IRS Form 990

• Including the signed letter from the CPA conducting the audit,

on the CPA firm's letterhead

• In compliance with Generally Accepted Accounting

Procedures (GAAP)

Reviews and compiled audits are not acceptable.

Required Documents: Names and DBA’s

If your organization goes by a name other than the Legal Name registered with the IRS:

• Provide the certificate showing the ‘Doing Business As’ (DBA) or Trade Name from the state of incorporation

Maryland – Department of Assessments and Taxation

Virginia – State Corporation Commission

District of Columbia – Department of Consumer and Regulatory Affairs

If you have a new legal name, you must submit a new IRS Letter of Determination (and documentation of the change)

Avoid Common Mistakes!

• Submit early and before the deadline

• Name and address in application must match IRS docs

• Program descriptions and dates are brief and specific about

the actual services provided for the 2014 calendar year

• IRS Form 990 must be SIGNED and dated by officer

• Number of board members on page 1, line 3 matches number

of trustees listed on pages 7-8

• For EZ or N (postcard) filers: ensure all required sections of

the proforma are completed
